This could've been neither of you actually. I went against that perk a couple days ago, I'm absolutely sure I hit all the skill checks but it still blew up and I was alone on the gen. I think it's bugged.

@@neonxrr Whether or not she actually missed it, it did blow up for her. But yeh MS is awful rn, you can hit it dead centre and still fail.
