El título de la canción remite al mundo del teatro anglosajón. "Send in the clowns" es una frase usada en el teatro anglosajón para referirse a esos momentos incómodos en los que la audiencia no responde como se espera, "manda a los payasos" significa, por lo tanto, hacer uso de recursos que nunca fallan y así la audiencia se anime.
En el caso de esta canción se refiere a ese momento incómodo de la vida de la protagonista (Desirée) que en el momento de la escena es rechazada por un antiguo amor de la juventud (Fredrik) que años antes le había propuesto matrimonio y que incluso tuvo una hija de él, sin embargo ella lo había rechazado y ahora ella resulta ser la rechazada.
