The entire time Lena was manipulating Kara, Katie made the acting choice to show doubt. Lena hated doing it, every bad choice she made she hated it. Whenever she made Kara smile she felt horrible knowing it was a lie. I always felt like Lena convinced herself she was just venting. I think Lena fully intended on fixing things and thought Kara would be there when she finally calmed down. 5x13 crushed her, actual tears in her eyes when Kara told her she could no longer turn a blind eye and that she would actively pursue stopping Lena. I think Lena doubled down not out of anger, but desperation to prove to Kara she wasn't evil. If she could make her plan succeed, then she could fix things with her head held high and show Kara she never turned bad. Unfortunately Lex showed her that the moment she shook his hand, she went villain.
The show never fully acknowledged it, probably cause they had to cut season 5 short, but what Lena did was so much worse than what Kara did. Lena was able to watch Kara day in and day do everything for her, work tirelessly to fix things, knowing that she was just manipulating her. She was able to intentionally crush Kara and cause pain time and time again without remorse. Then the one time she shows concern she flips out on Kara again the moment Lex says something. Lena never EVER truly apologized for mentally and physically abusing Kara. I think in my head Lena knew that because of what she did, she could never be more than a friend to Kara. The lengths she went to out of anger destroyed any chance of that and I think Lena realized this as Kara expressed her pain. Lena came to 2 realizations during Kara's speech. First she realized that she had been wrong, that Kara had cared about her as much as she did for Kara. She saw the pain in Kara's eyes and realized in that moment that she had succeeded in crushing her and it broke her even more to know that she caused that pain. The second thing Lena realized was that she lost the chance to have everything she had wanted. She lost the chance to have the love of someone who would do anything for her. You could see in Lena's eyes as she nodded her head that she accepted that she destroyed her own chance to be loved unconditionally. I like to think that during season 6 Lena set out to insure that nobody could hurt Kara the way she did. She became her guardian, both physically and emotionally. She never let Kara go down a dark path, she stayed close to her and kept her heart safe. Lena may never be able to be the love Kara deserved, but she would never let Kara get hurt again, I truly feel that way. That's not say that Kara agreed. In my head cannon, once Kara accepted her own feelings for Lena, with Lena's help, Kara would help Lena overcome her fear of hurting Kara again.
The writers were never able to make Supercorp Canon, though they clearly wanted to. So instead they used double meanings in everything. One of those hints they sent was Andrea Rojas. They made sure we saw Lena had feelings like she did for Kara before. You can't tell me the Andrea thing was am accident by the writers. They used parallels in that entire relationship right as Supercorp was at its largest in popularity. This was to tell us Lena had deep feelings for the same sex before. So as a Behavioral Health Professional I have a breakdown that I think Supercorp fans will appreciate. Lena broke the way she did because the things she was hiding were brought to the surface and not just her denial of who Supergirl was. I'm not saying the show was implying Lena realized she was gay, but they were implying Lena cared for Kara in a way that went beyond friendship. Lena's pain was that she thought Kara didn't care as much as she did. Then Kara did her own speech and Lena realized that Kara was in as much denial as she had been. Season 6 is kind of proof of my theory. Season 6 was primarily Lena helping Kara except herself for who she is. Now this can be taken multiple ways and that was intentionally done by writers who weren't aloud to make them blatantly Canon, so instead they used double meaning on almost everything in season 6. Lena knew before Kara how she felt, that was clear by how different Lena became after she realized how she felt. The rest of the time was her patiently helping Kara come to terms with her own feelings and you can see Kara slowly except these things. Denial can be very very powerful. But if you look, Lena had slowly taken Alex's place in Kara's life and even Alex herself noticed this. You can't tell me Alex would feel okay in letting just a friend take her role in Kara's life. No she felt safe in leaving Kara with Lena cause she saw the connection, the writers made sure we knew Alex knew. All of this was intentionally made apparent to us that Lena had become the most important person in Kara's life and that last scene was Lena telling Kara she excepted herself and all that she is and that if Kara did the same, she would be there for her. Kara taking off the glasses while staring at Lena was a symbol from the writers of Kara excepting herself and an expression of what mattered to her, Lena Luthor.

About Lena taking Alex's place and they did it slowly, for example when Kara wakes up after the Phantom Zone only Alex is there, but when Kara wakes up because she passed out Alex and Lena are there together and then when Lex hits her with kryptonite Lena is there alone with her when she wakes up.

When Kara returns in 6x7, the show makes sure we see Alex notice the extended hug between Lena and Kara. Now there are many ways this cam be taken. In season 6 I had a hard time with how little they focused on Alex and Kara. That relationship was the show in my opinion. But then I started to see what they were trying to relay to the audience. Without coming straight forward and saying it, they were showing the evolution of Kara and Lena. Alex had finally done what she always wanted, found a life outside of Kara, she deserved that. On the other hand Kara, without realizing it, found someone worthy to take her place. Someone to eat potstickers with, be her perfect partner on game night, and willing to really listen when she needed it, Lena replaced Alex. I don't mean that in a bad way, but that's why the show displayed Alex as unwilling to really give Kara advice in their final scene and instead said it's something she had to figure out for herself. Alex had a well deserved life now and knew Kara had found someone worthy of her role in Kara's life. Lena on the other hand did give Kara advice. Not even Alex or Cat Grant said words that made Kara make the decision to take off the glasses. Lena Luthor is the one that got Kara to take off the glasses by telling Kara she would always be there no matter what choice Kara made and that opening up as her true self would be empowering is what encourages Kara to remove the glasses. But more than that, no matter what the ending said, Kara and Lena wouldn't be developing a life outside of each other like Alex ended up doing. I truly believe they would develope a life with each other.
