Try to use 5v dc from pc or 12v dc form car battery it fixed the issue for me .. at first i was using an adjustable power supply

You can use any speaker amplifier at least 5 W
Works, but sounds like crap. Use the circuit from the datasheet. Add a capacitor on vcc and ground to filter any noise, from your power source.
