You mentioned Prednisolone to be coded as '6' but just noticed you have written '10' with your sign and date

Why did you give the paracetamol? It is incomplete prescription. There is no start date and duration.
PRN medication no need to have stat date and finish date it's PRN
Check drs signature and dose for validity
