Amazing lecture as always, Dr. Morton, My only suggestion is in regard to your example, as you know better, spinal cord in adult end at the level of L1-L2 so we only have Lower motor neurons pathology if the damage is below L2 (conus medularis) and after this level, only Lower motor neurons run as cauda equina moving down through spinal canal to the respective levels for these neurons to leave the spinal canal and become the prespective nerve. othewise excellent presentation as always. I am confused at your question at 14:45. The spinal cord in an adult ends at around L2 so how can you have the sections of the spinal cord going down through L4 and S4? Lesions at the L4 level should be all LMNs. Can you please clarify this for me?
The reason why is that the question was asking about spinal cord levels and not vertebral levels. The spinal cord ends at the L2 Vertebral level. However, the spinal cord still possesses all segmental levels (8 cerv, 12 thor, 5 lum, 5 sac) ... these levels are just compacted into the space between the C1 and L2 vertebral levels. If this is still confusing go to 3:01 on my Segmentation of the spinal cord video (th-cam.com/video/vbZZigN9qKQ/w-d-xo.html)
Can you please explain why the right side of the body is lesioned and not the left for the first practice problem, I thought that since the corticospinal tract crosses over it would contralaterally affect the body
