My mother is a patient of ankyolising spondylitis. She is on tofacitinib. But from last few days she is feeling pain in the right side of upper back after long hours of standing. And feel relief after lying down. Is it related to AS or it's a mechanical pain?
Looks more like mechanical pain as it increase on standing & is relieved by rest. However, one needs to examine for confirmation.
@@DrShashankAkerkar it's also increases after prolonged sitting. We just tested. The crp is 5mg/L and esr is 21mm. Told my rheumatologist about this issue. But he didn't care much. Just told to continue excersice. Wondering is it due to as or other reasons.

Sir ,is it possible to have AS without raised in inflammatory markers ,my CRP never crossed 1 and ESR remained 20 in last one year ,should i go for MRI , previous MRI was 100% normal
It can happen; however the MRI is expected to show inflammation if you have had backache. If one does not have backache but only pain/ swelling of peripheral joints (knee/ ankle/ shoulder etc) then back MRI would be normal.

Sir can you please advice why different rheumatologist have different opinions regarding usage of Sulfasalazine in Ankylosing Spondylitis.
I have visited 2 doctors
One of whom has prescribed sulfasalazine for regular basis and etoshine on sos irrespective of where the inflammation is in the body.
While the other doctor said that sulfasalazine doesn't work if inflammation is limited to spine and spine connecting joints, he says Sulfasalazine works when inflammation is outside spine like ankle,knee,wrist or shoulders and have prescribed etoshine twice a week and Tizan 2mg once daily.
Please can you suggest your valuable inputs.
This difference of opinion does exist. A few studies have shown that sulphasalazine does not work for inflammation limited to spine & SI joints. Hence many rheumatologists are of the opinion that it does not work unless there is inflammation of the peripheral joints. There are also a few studies that have shown utility of Sulphasalazine in inflammation of vertebral joints & SI joints. We have been using sulfasalazine for many years & have found it useful. Hence a few rheumatologists believe it works & are using it.
