If the cable has shielding underneath the insulation, how should we proceed? Does it need to be cut off?

Hi there! If you're comfortable with the shielding being connected to the penetrator bolt and water, you can leave it as is. Otherwise we'd recommend putting a thin layer of heatshrink around it before installing the penetrator to keep it isolated.
