  • Modafinil / Provigil garnered FDA approval as therapy for excessive daytime sleepiness related to narcolepsy, obstructive sleep apnea and sleep disorders associated with rotating work shifts. In spite of these relatively limited authorized indications, more than 90% of prescriptions for Modafinil / Provigil are for so called "off label" indications.
    Modafinil / Provigil remains an unexplored but popular therapy for Parkinson's disease, schizophrenia, bipolar disease, depression and chronic fatigue syndrome. Among the other popular uses for Modafinil / Provigil are ADHD, fatigue associated with mood disorders and lethargy resulting from antipsychotic drugs.
    Modafinil / Provigil gained popularity as a "smart pill" supposedly able to delay sleep and allow increased focus on school or work projects. Even the armed forces approve its use for troops on missions requiring heightened vigilance with little time for sleep. Unlike some pills Modafinil / Provigil does not impair judgment, thinking or motor skills.
    Recent warnings especially from the European Medicines Agency - sort of the FDA of the entire European Union - suggest caution with Modafinil / Provigil. Reports link it to a number of life threatening side effects including multi-organ hypersensitivity reactions, heart rhythm disturbances and sudden cardiac death.

      Yup, I really admire him. Most of the doctors I've come across just know the names of medicines. Even some of them don't know the mechanism. A friend of mine is a G.P and he's really not knowledgeable.

    Modafinil helps my depression, in combination with an SSRI, more than anything in all of the years I’ve tried to find something that would work. I’m past menopause and I’m so so grateful for Modafinil.

      @@pennybabyboo If you’ve already tried a lot of alternatives for depression , I recommend talking to a psychiatrist who could work with you on alternatives such as modafinil. I also had sleepiness and ADHD so that played into my diagnosis and my doctor’s decision to try modafinil as an adjunct to an ssri . Sometimes it takes a cocktail to work. It doesn’t work as a stimulant on me, nor a super-focuser.

      Me too. After going thru 7 SSRI potential solutions, Modafinil made the difference. That's all I take now. I guess it allows me to focus and 'put in the side mirror' the negative feelings I was trying to eliminate with the SSRIs.
      I find this report rather cynical. A tiny shift in your outlook can make all the difference. Modafinil could provide that shift. Worth trying before the rabbit hole of anti-deps.

      Perhaps your doc doesn't want to prescribe it off-label. If you told a little white lie and said you were constantly falling asleep during the day then you might have more luck as it is regularly prescribed for narcolepsy - at least here in the UK.

    Thanks for uploading this instructive video. You mention studies done on people playing chess @22:25. I occasionally take non-prescribed Modafinil for cognitive enhancement at my desk job and am a regular online chess player. I find that when playing chess, Modafinil will allow me to focus for much longer on one move when analysing a line (chess variation) - this always comes at the cost of more time spent on each move but yields better quality moves in general, and on long games a better match outcome.
    If I play 10 minute chess or less, the quality of the moves are less important, the advantage goes to the player that can make good practical decisions and this requires multi-tasking - which in my experience is hindered by Modafinil.

      I also tested that smart drug. To be honest I couldn’t notice anything. Chess is an addictive game. At times, I played that game for hours. No, I wasn’t on Moda in those days. I couldn’t believe myself. I played for more than 12 hours. I removed the game from my mobile. I didn’t want that game controlled my life. I only play that game if I have enough free time. Unfortunately, there isn’t any smart drug which can increase the retention of new information. You won’t become smarter. If you suck at maths, languages, drawing whatever, you name it, moda or Adderal won’t help you to enjoy maths. Those pills don’t give you super powers. Yeah alertness, your concentration will be raised, however, your IQ, Your fluid intelligence will be the same. You might boost your crystal intelligence. I am very disappointed. Calling those pills “ Smart drugs” is an insult. There are good for people who suffer from narcolepsy.

      This is essentially the same conclusion I reached. I would buy it online to experiment with - mainly for being more productive and getting through long work days on limited sleep.
      I found it made me more “disorganised” and scattered.
      Years down the road I was diagnosed with ADHD which some were surprised wasn’t discovered sooner. Currently being prescribed Ritalin, which I was concerned may give a similar effect to Modafinil.
      Ritalin seems to be more helpful for focus and feeling more “tuned in”. I don’t get headaches like I do on modafinil either.

      It's interesting since I have type 2 narcolepsy but I don't get the hyperfocus that everyone denotes to it. Been taking 200 mg on and off for a few months now. It just wakes me up. Somehow caffeine gives me substantially more euphoria than modafinil lol

    It does sound like you’re trying to discourage the use of Modafinil. Don’t virtually All psychotropic or even other pharmaceuticals come with a huge list of potential side effects? To me Modafinil is a lifesaver.

      Going to be asking my doctor if I can try this at my next appointment in September. I have never had a working brain. And I am almost 40. Pots , chronic fatigue and brain fog 😶‍🌫️. Can’t drive , never went to university. Can’t work. From my brain.

    Hello out there. I hope everyone is doing well. I would just like to comment on this topic and say I have severe ADHD and I have been on one form of amphetamine or another since second grade. I’m now 51 I just got off of the new version of Adderall, the one in the last 16 hours I was on the maximum dose, and you know I finally just decided I was done the benefits or fading. The negative affects were increasing. I’ve been down the road of amphetamines. I know where it leads it always leads to that same ugly place you know it might take certain people different amounts of time to get there, but they all arrive at the same ugly place. I’ve been researching modafinil and I took my first one yesterday and oh my God it’s the closest to normal that I have ever felt I wasn’t and up like with him that I mean the brain fog cleared. I had an underlying sense of happiness and well-being. My thoughts were clear and not racing. I was actually able to Pull a thought down and you know work with it think about it analyze that and then Move on. I’ve had no headache really no issues whatsoever. I just feel like a normal person. Having an average good day. That is how I would describe it it’s not like how was your day. Oh my God I want a car kind of thing that is amphetamine this is like how was your day I was pretty good I was really productive. I got my project finished on time. Had a nice lunch. Traffic wasn’t too bad you know that kind of thing that’s also another reason it’s not something you would abuse because you cannot double up on that kind of a feeling or you go into oh my God I want to car kind of a thing that’s the realm of amphetamines as I said before. This is more like a natural, gentle, uplifting, not a jaw, clenching mechanical drive towards nothing in particular it’s not that kind of thing at all is there was ever time in your life when you were younger, and it was just easier for you to think and control your thoughts, and you were generally excited about life and happy, that’s kind of how this feels. It’s not like a big rush of dopamine. It is actually everything I hoped it would be and more. Do you know what’s really enjoyable? To smell marijuana with modafinil that being said the one daffodil I’m noticing is taking away my urge to smoke marijuana, which is something that I have struggled with quitting for a very long time. But when you start to gain control of your thoughts and you’re not on edge and you can actually just like relax and think for a minute, there’s really no reason to have anxiety. Let me also say that I take Xanax and have taken that off and on for years this is coming in a completely different way, it’s just like you are calm because that’s just the way you are in the mood you were in and how you always are it’s not like you feel this sedating relief of an anxiety attack going away because you’re being sedated by benzodiazepines. So if you are someone that is struggling with ADHD, struggling with amphetamines to treat ADHD, feeling like amphetamines are exacerbating the depression and sometimes the mental fog.
    I am a runner, and I noticed when I didn’t take the antenna means it reduced my ability to run, and then I noticed even when I was taking them it was reducing my ability to run it took away my speed and my distance. I am giving myself a good five days of just adjusting to modafinil and taking it easy, letting my body recover like my mind recover from the constant push from amphetamines. I may do a few easy rounds, but I’m not gonna push or try to improve speed or distance until my body signals that it’s ready.

      Well I can just say that I have severe add/adhd and I have tried modafinil and almost any other medication for it but the only thing that makes me feel normal is dex amphetamine or methamphetamine

      @@samuelsales401 For me: Pros:1 . Took away anxiety. 2. Gave me confidence. 3. Gave my constantly fatigued butt energy. 4. Focus for hours. 5. Got stuff done.
      2. A) Cons: After a week or so, effects level off. So you increase dosage. From 100 to 150...then taking a full 200mg pill. But it all levels off. Still kinda works. But instead of being brain rocket fuel, it's more like a strong cup of coffee.
      B) Withdrawel. Doc above says it doesn't. But for me I'm especially tired without it. Which is why I've stayed on it for 3 years at 200mg daily. But now my prescription ran out and I can't find my doctor. So I guess I'm going cold turkey. I hope I don't get so tired I never wake up again.
      I feel it's for the best. I've tried to cut back or get off it for years, but always fail. I think I'm done with this thing. One less thing to hassle and worry about. So be it.

    Most people who take it for off label uses, use it during the day to promote focus, not at night to avoid sleep. Most of them say the same thing, take it in the morning and you should not have trouble sleeping when you are finished with your day.

      @@serious_killer1653 why would it? It has no direct effect on oestrogenic receptors as far as we know which are what stimulate the premature growth plate closure. I mean it's possible increasing norepinephrine as it does may affect hormones but I'd doubt it would make any noticeable difference. Adderall has a much more intense effect on those transmitters and we haven't seen noticeable changes in height with adolescents taking it so one can assume Modafinil is safe in that regard. Obviously we can't be positive given it's relatively short history though.

    It’s must be good for us... as many videos go and on about negative side of it.
    I take 1/4 pill 1X - 2X a day 3-5 days a week and skip a week here and there.
    I love it over coffees and teas...
    I find I’m way more animated in the pill and this equals being a little sore the next day as I act out physically, ride my Harley hard... and have a semi physical job which I do with more fervor in the pill.
    I get anxiety on 1/2 a pill.
    I do deep breathing and walking and time in nature.
    You’ll find your courage growing too, which is a nice touch. If you take this prior to social experiences and are in basic mood... you may find yourself way more up and animated / on point.

    Very interesting. I have it for narcolepsy. at a cost of $5.60 per month. I live in Australia where the government heavily subsidised. Doc said I can take a break from the drug on weekends so I can minimise the side effects. I get skin rashes which are so severe that the skin breaks . It does give you tight focus, I wish I had it when I was studying. I was diagnosed when I was 55. Angry that I lived most of my life with this condition. When specialist first saw me, he later said that he knew that I was narcoleptic. He admitted to hospital immediately. Apparently, I can do a full 90 minute sleep cycle in a matter of 14 minutes and I do it 4-5 times a day. I’m a robot, rather than the fall down type of person. I just keep doing things. Humorously, one day arriving home , I put my glasses in the freezer and my handbag in the oven, whilst in a narcoleptic state. So I can say the drug definitely helps me manage my condition. Also, you shouldn’t drink alcohol when on the drug.

      Thanks for your input! Interesting indeed. I'm sorry that you have narcolepsy. I'm an Australian medical student. Is your skin problem Stevens-Johnson syndrome do you know? It seems that narcolepsy is an autoimmune condition where cells in the hypothalamus (I think - or pituitary) are attacked by autoimmune processes. And I think this involves orexin (I'm sitting in the garden so this is off the top of my head). All the best.

      Spot-on. This medication can make some people including me extremely drowsy and causes severe depression. It gave me a lot of physical side effects. The first one was headaches which is a famous side effect of this medicine. Most of the times, the day after taking it I was too exhausted( many people experience this) and it's a medication which every time which I took it, the result was different. Once I got energetic, so many times it made me so sleepy. Just awful

      That's very common with stimulants if you are using them to stay awake. There is no substitute for sleep.

      A lot of people experience irritability, that's why some psychiatrists prescribe Benzodiazepines with stimulants as well to take the edge off.

      Justin Boger Adderall maybe a bit too much for kids because it’s more potent than modafinil. look up the active ingredient for both.

    I take it, It has pros and cons. If you take it wrong you lose sleep, The the next day your tempted to take more which leads to more sleep loss. Military found they could keep souldiers awake for 72 hours with minimal effects on cognitive performance when using 300 mg a day. The average recommended dose is 200 MG but with a half life of 12 to 16 hours, depending on your metabolism it might throw off your whole system. Taking it when your already sleep deprived isnt as effective as taking it after a full night sleep. Id say an ideal dose is 150 milligrams..

    Part of a Smart Stack. No side effects or dependency fears. If taken too often, then a tolerance will develop that will encourage one to stop temporarily. 3x/week about good.

    Lots of good info. Thanks. What are the serious side effects? How can it be recommended for pilots and astronauts if it can cause death? How often do these side effects occur? So many benign drugs can have bad side effects in rare cases. And obviously a stimulant will reduce the effects of an opioide. No surprise. You should give percentages. If one in 20,000 have bad side effects and because of known problems then it's safe.

      No percentage is known - as far as I've read, the mortal side effect is Steven's Johnson Syndrome, and it happened in one person during one of the studies.

    I find it is the only medication that helps me deal with M.E. daytime exhaustion. Literally nothing else works, so why is it not made available for this treatment on the NHS? It has only upsides and no downsides for me, yet I still have to find it online.

Your being a Medical Professional speaking to a general audience about a drug that has the tendency to abuse, I can understand where you are coming from in the delivery of your message. My small critique, if I may, is on behalf of those who are sincerely seeking healthiest brain chemistry. These minority cases are being given far more representation in the video than the greater patients who report benefits. Qualifying the negative effects in conjunction with monkey studies taking cocaine but de-emphasizing the benefits with "there's no evidence of tolerance or withdrawal, but..." I completely understand; though, what of detouring individuals with diagnosed illnesses who would bring this up with their attending physician?

      @@wellnowdoctor5976 continued 2.... I have had my ADHD tested in private health, the Veterans Hospital, and twice by evaluators assessing cognitive ability and learning disability. Though an off-label treatment, anecdotal evidence appears positive toward ADHD treatment and tertiary benefit report improved outcomes in cases of depression and anxiety. My Adderall XR dose has grown to 40mg/day. Any lower and, not only is it not effective enough, but I have about 2 instances of sudden sleepiness throughout the day. Adding Modafinil might allow me to reduce this high dose of Adderall, reduce dependency, and hopefully result in a far safer treatment profile. Thank you for your time and ear.

    This guy is basically repeating the same thing over and over again the only real damage that can come from modafinil is a skin rash which is extremely rare and about equal to pretty many other medication on the market. He does not mention that it has a little effect on the dopamine system it’s considered non-habit-forming, and that is primarily works by stimulating or acts in a protein that activates alertness rather than norepinephrine. I’d be surprised if this guy didn’t work for the manufacturers of Prozac.

    Carisoprodol is no joke!!! That’s a crazy muscle relaxer that if misused with alcohol will cause a temporary blackout yet you are still doing a task. I do not miss that drug what do ever .

    I found it to work excellent for treating my ADHD. However, tolerance builds so rapidly it isn't a decent candidate for long term use at least in my case. I do use it periodically when Adderall is on backorder at my pharmacy or if I'm taking a break from my Adderall for brain health. It's very obvious in me personally that Modafinil stimulates dopamine much less than typical wakefulness drugs. That pleasureful need to redose doesn't exist with modafinil. I also have found it almost useless to abuse as in taking more doesn't produce any pleasurable effects nor does it seem to do much at all above 400mg. It did give me bad headaches at first but only a couple times. I never knew it can block hydrocodone/opiates but that does make sense as I was prescribed and I noticed the pain reduction was diminished greatly.

    Buprenorphine will this potentiate the analgesic properties of subutex?
    I'm slightly worried. I do find it works well. But I only take it when its rearly bad for me. I'm prescribed 200mg but take 50mg that suites me. To get me going. U gotta saty hydrated on this material I find.

      I do and it works better for me although I have to take it at 4 am, I fall back to sleep until 7:30/8:00 am and when I wake up I am really motivated alert and present. If I take it too late I don't sleep that night. Ritalin is better to take later in the day and you can still sleep. Modafanil sort of doesn't feel like anything at all but you'll notice it when you don't have it. I have Complex PTSD that causes ADHD like symptoms. You can focus intensely with modafanil which is great except if you get distracted you will be intensely focused on the distraction. Take it with a plan. Take it early and get alot done so you can sleep at night. You will feel the dose if you take 200mg but its better if you only take 100 mg ( you wont even feel it ) but you will still have a razor like focus. Its not a magic bullet but If you have ADHD it is very helpful. Generics seem to have inconsistent strengths from tablet to tablet.

      It’s helped me and I love a holistic life.
      I use it 3-5 days a week. And I skip a week here and there for comfort and chill.
      I go 1/4 pill and like it.
      Morning and noon.
      Like a 1/2 pill only a little spread out.
      Gives me confidence to engage in many things. My whole life needs a upgrade... this is helping me do that.
      I want a better base. This is helping me gain that... then I plan to back off and use more for special occasions.
      But I use allot of herbs, mushrooms, vitamins... so idk 🤷🏻‍♂️. This is another tool, but highly noticeable in short order.

      Small doses are helpfull, Large ones keep you awake too long. The focus window is like 7 - 10 hours but if your awake after that your like a zombie.

    When I took 200mg, the next days I was sleeping throughout the day, I was falling asleep in just minutes which is completely abnormal for me. I was sleeping 16 hours per day. 100mg is fine and has some effect for me but nothing too crazy, the effect is minor. For most who take it, seems to be just psychological.
    Only other side effect beside lethargy for me is nausea, which can be bad.
