It's an interesting concept. Usually in this type of isekai story, the world you enter into is as real as ours even if its origin comes from a game, and the people are equally real. But in this case, she apparently started out with the assumption that they were real people and tried for 20 years to see what is behind the basic NPCs she knew from the game, only to find that there wasn't anything there.
The idea that all the people in the palace where just super shallow is funny, but you could also consider that being put into a station of high authority at a young age made everyone intimidated by her leading them to hide their personalities out of fear of offending her somehow.
This reminds me of the episode of the Sword Art Online Abridged series where the guy keeps a bunch of NPCs from the tutorial quests to help him in the game. They kept spouting off lines from their quests at random intervals since they were NPCs. Stuff like "we must save my family!" and "the bandits are coming!" multiple times. If that's the kind of thing she went through, hearing every dialogue option over and over for 20 years, that would be truly depressing. To finally have someone she could actual talk to and have a real conversation about real things with, I can't imagine the relief you'd feel from that.
Damn, this was a great script! I personally believe the "NPCs" aren't fake the way this villainess sees them; she just had the misfortune of entering this world with that idea and therefore confirmation bias, plus being born in to a royal setting where people are pretty notoriously 'fake' or shallow in personality. I'd love for a continuation where she joins the hero's party and goes on adventures and is shown that the people of the world are real, but also it would be heartbreaking to hear her suddenly realize all the awful shit she did was real too. (Assuming she's only so indifferent to that suffering because of the NPC thing and not just an actual complete lack of empathy carried over from her previous life)
I've seen at least one manga that kind of played with this, with the antagonist stubbornly refusing to acknowledge that the world wasn't a video game and trying to force square pegs into round holes.
This reminds me of when I was younger and I played "Silient Hill 4: The room" for the old PS2. I was so drawn to an NPC that when the main villain killed her in the first 1/4 of the game I became LITERALLY enraged and played the game non-stop for 12 hours until I killed him and completed the game. This is a really great concept, the clash of ideals between the Villainess and the Hero in how they treat the NPCs was a great touch. I hope this wasn't a one-shot because I'd love to hear more.
Y’know, it could be equally as true that as an individual born into power in a new world the people at the palace could be extremely fake. Part of me wonders about confirmation bias. I would recommend both the listener and villainess going incognito and seeing “nameless” people from both countries cause it could be that she’s right, and all the protag vendors have a smidge of personality and background while hers are just “hi, I am smith, would you like buy?”

I would love a series of this maybe a like immediate "I... I need time to think, just let us go and I'll figure things out" and then it ends up not being enemies TO lovers but rather enemies AND lovers. She said it herself, ruling is dull unless she's being opposed, so he wants to oppose her publically, leading armies against her trying to outsmart and outmaneuver eachother only to call peace meetings to hang out and maybe some... lewd handholding

Been thinking about this audio constantly these last couple days. That comment about the people in the palace just being extraordinarily shallow and not actual NPCs has been making me think... what if the listener's party are all real people with real emotions but the queen starts getting to the listener, convincing them that their friends aren't real? Can you imagine your friend or worse, lover bringing your greatest enemy along for the ride after you've dethroned her? And how weirdly obsessed she is with them? The more they talk to her, the more they treat you differently. It's almost like they're testing you, trying to get a rise out of you but nothing you say seems to be the right answer. As time goes on they only speak to you when they need something, and any personal time is reserved for this woman they barely know. That they loathed a few short months ago. And you have no idea how to handle that. You, quite literally cannot separate them. And it's becoming apparent that sooner or later the wedge driven between the two of you is going to become permanent. Idk, I just have been thinking about the horror of the person closest to you becoming convinced you're not real a lot lately.

I'd recommend skipping it and go for something like Solo Leveling or Overlord. Sword Art Online was better when I first saw it, but coming back to it makes me see all the flaws in it. It's still infinitely better than the king of cringe, known as eminence in shadow. Seriously, DON'T WATCH IT!!!
