You gotta turn up the user and cpu injury sliders frequency and severity . Along with the career threatening slider , yours just say career threatening only
Tip for Peter: If you are gonna make this video or an injury video again then do the same thing with settings like you did but go to cpu sliders and turn durability to 0.Another tip is to look at morale difficulties and there is a cpu or user injury effects(to make injury’s more severe) and cpu and user frequency.
