Tomoya had given his heart to Ayane, but his uncle (Hitoshi) and his aunt (Momoka) showed no remorse because of Tomoya's status and using illegal tricks to do the heart transfer. However, Ayane didn't like trick and hates her parents for this. Even though they claim to be elite, they are not above the law. The same goes for Kunio. He is supposed to be in a profession that helps people, but all he wants is money and would cut corners to earn it. In the end,they got both criminal justice and karmic justice for being cold-hearted to Tomoya and Shigeru.

I can’t believe that Sakai’s brother would kidnap Tomoya and lied to the doctors only to give Ayana a better heart transplant. Ayane told the truth about her parents and one of the doctors using forgery to ruin Sakai’s life and his son. Mr Moroboshi saw everything so Ayane’s parents were arrested for felony and so as the doctor and karma has hit them. Tomoya is now recovered from the surgery and Ayane is living with Sakai and Tomoya arms they are going to have a happy life. Parents should never lie behind their children’s back and even looking down to their own family members. Also, kidnapping them and even taking advantage with medical professionals and forgery can lead you to jail. If you really need your child have a better surgery you need to work very hard to save money and be honest.
I don't think there's no need for that since they're pretty much all just university students that are attending a Dojo. Their bio would be all the same if they did that. The only slight difference is if they attend different schools or work part time somewhere.

Meanwhile, Ayane had her own new heart that was made from her own cells, and both she, and Tomoya, have no scars, on the outside, or on the inside, of their bodies. Eventually, the two kids grew up, Ayane found the man of her dreams with her best childhood friend, who was always by her side, and she married him once the two of them graduated from the same university together. Likewise, the best childhood friend of Tomoya also visited him, fell in love with him, and married him once they graduated from their university.

Like the story but a bit of a critique here. At 7:50 moroboshi says using a heart that doesn't have an diseases isn't allowed. This is incorrect, the organ needs to be healthy and uncomprimised in order to be used. If not you risk contaminating a person to something they may not already have.
