    thank you so much Dr tala ,your videos are so helpful for me, I am a neonatologist from Egypt , I have a question about lipids in TPN...Is there any association between sepsis and lipids? shall we not add lipids to TPN incase of sepsis?

      Hello! So happy you’re finding some usefulness in these videos! This is a question we’re always asking ourselves in the unit. We know lipids affect how wbcs and platelets function in the lab- and that in sepsis babies are more likely to get high TG from same level- but usually we continue same dosing through sepsis treatment. Under the logic that we want good nutrition and all the good fatty acids when we’re sick. What do you do?

      In our policy we discontinue and stop lipids from TPN when sepsis occur​@@TalaTalksNICU

    Hi! Thanks for this very informative video on TPN .
    Im a pediatric trainee from Malaysia
    Would like to get your opinion if the amino acids in the TPN would affect or contribute to metabolic acidosis in a newborn prem baby? Just wondering if it could be one of a cause of persistent metabolic acidosis after ruling out sepsis, dehydration, anemia and pDA.

      Hello Rachel- this is a great question. theoretically AA can cause metabolic acidosis- but this is less likely than other causes. Not sure if you have cysteine in your TPN- this is more likely to cause acidosis. Most likely (if other more pathologic causes have been ruled out)- it's immature kidneys dumping out too much bicarb- and leaving an acidosis. We need to discuss this more! Thanks for your great question :)

    The extremely high energy he got is directed towards gaining weight as well as basic metabolism
    imagine 60 kg adult who adds 10g/kg daily
    ..that is 600 gram/d or 18 Kg every month...horrible😊
