The real issue now is that the boys are doing this mandatory service for the nation, that’s why we need more responsibility and accountability from the government. Only due to the fact he’s an actor and somewhat an public figure, the issue was magnified if not it could be just another “case” simply sweep away without much details known to the public.
The government has formed a COI to investigate the matter. If it is negligence and non-compliance on the officers' part when there is protocol already put in place, honestly it is unfair to put the blame on the government. If it happens to a few officers not following procedures, you can't generalize to say that the whole army is at fault isn't it. Hopefully due to the public nature of this case, there would be more awareness of the importance of safety and compliance.

But this is straight up a negligence and insubordination issue. The victim broke the chain of command whilst the commander didn't either secured the situation or act rightfully according to SOP.
So, train the soldiers better to avoid this horrendous incident in the future.

In that case, the protocol or the army is at fault. In the context that this is a noisy environment as expected, you can't just shout "Clear" and exonerate yourself from responsibility, from actually making sure that the area is in-fact cleared as this is a matter of life and death of your own comrade. Also I find it plausible that the sequence of events could be inconsistent. How much time has elapsed from the technician acknowledgement to barrel being lowered? The acknowledgement could have been superseded by time of events.
When there's severe injuries in the US they try to mitigate deaths by responding and getting the victim to emergency surgery in the golden hour period or 60 minutes. For example the 5.56mm round yaws and tumbles in human flesh and causes a lot of damage but with advanced surgery and emergency care, survivability is greatly increased when rushed to hospital in 60 minutes.
I think for Aloysius' case, 3 whole hours is really unacceptable before he was evacuated especially when there is severe trauma to the injured soldier. What should have been arranged was an immediate medivac or helicopter evac of the wounded on the site of the event occured. This should all have been planned out.........
So now you know how dangerous combat vocations are. And the sad thing is people in combat vocations are not appreciated but treated like dogs

Combat vocations are dangerous. It is also the NS men own responsibility to follow and comply with the TSR at all times.
Sad and sorry to say, in the case of Aloysius Pang here, he did not heed the safety instructions. He compromised it. Hence, he is partly also to be blamed.
The clerk interacts with the CO all the time and gets best soldier of the month. Meanwhile the combat soldiers on the ground are just like pawns to be deployed.

Safety rules are set for a purpose - save lives. Once breached, one will be endangered regardless of who you are.

It was more of the fault of the commander than Aloysius. Yes Aloysius was wrong too but the onus lies heavier on the commander to make sure that all his man are safe.

We should make it a protocol that when this action is initiated, commander hand must be in position to push the emergency stop button and in full view of the entire barrel.

Emergency button is wwaayyy out of reach from operator. This is why. Emergency button should be installed on the control panel or nearer to operator.
They should make the safety stop on ground as in whole platform of danger zone. Like if floor of unsafe area is step then the gun thing will not operate. I see this as design flaws and human error. The floor should be mark by color of where exactly is safe /not. Also have reminder placed inside the tank. Just because train does not mean they remembered. Also at time of panic who will think that straight? I hate the way it is shown as push blame

I am skeptical if the dialogue really took place. I think all three of them just focused on doing their own thing and led to the final outcome.

In this case, all the officers in the vehicle seem to have some form of negligence and non-compliance in carrying out the exercise. It is everyone's duty to ensure that safety procedures are adhered to, no matter how much they think they are "safe", and also on the commander in charge a responsibility to ensure that all members are in safe positions before activating the exercise. Perhaps some lessons to learn is not to take these protocols lightly and also understand the utmost importance of safety, follow instructions and embrace the responsibilities of their positions at hand, no matter what background the officers came from. As comrades, they should also look out for one another and be brave to flag out issues when there is one. Hopefully due to the public nature of this case, there would be greater awareness of the importance of safety and compliance in carrying out these exercises.

Horrible design of this War machine. Shows how much thought the SAF puts into the safety aspect of it’s servicemen operating it before procuring them. One would have assumed that a dual modular safety redundancy would have been incorporated into it’s design…
If such serious accident could happened during Peacetime, it will CERTAINLY happen AGAIN when chaotic moments happen during wartime!

I'm sure the victim's family was informed of the investigation. How would you like it if this were to happen to your child and everytime you look at the news you are constantly being reminded of this tragic incident.
