Depression. She may not see how bad her surroundings have gotten if she's literally given up on life. Cleaning requires motivation and physical energy, things depressed people can't summon up, regardless of how disorganized their homes become.
Hoarding is an entirely different situation; I don't know enough about it to comment, but it's a very real disorder and difficult to treat.
