I find that this plays closer to the NES version, than the SNES version where enemies seem to be more aggressive, and during Stage 2's bike segment you could no longer hit from the front of your leg.
I've played the SNES version, and I can say that mechanically it's pretty garbage, compared to original and on its own. The Sega port is way on another level. It's very close to original, and pretty solid game overall.
