You need 6 foot long stalls for the average size holstein, also a 48" high head rail set forward enough that the cows can't push on it with there neck.
Docking tails is not necessary and prevents cows from dealing with flies. The only reason to dock a cows tail is for the farmers comfort. It should be dealt with as animal cruelty.
I am not so sure they are bigger now , We milked around 40 cows back 14 years ago and the cows I see now are smaller built smaller legs and frames , we had cows that were 1,500-2,000 now many I see struggle to get to 1,200 ,I guess it comes down to what bull the farmer uses
