Basically I have my iPad sitting on the edge of my desk on a nonslip mat, keyboard pushed up against the top for even better stability, I put my phone holder arm to the left, have the phone angled correctly. As for lighting, I open two fully white websites on both of my monitors for a white backlight (this works very well). On my phone I wipe the camera lens to remove blur, and balance the brightness to favor visibility of the screen without making my hands entirely black. It's a little lengthy but it provides pretty good results most of the time
