The QRS complex is an important component of the ECG that represents the electrical activity of the ventricles of the heart. The duration and morphology of the QRS complex can provide valuable information about the heart's electrical conduction system, and can help diagnose various heart conditions such as bundle branch block, ventricular hypertrophy, and myocardial infarction. In this tutorial, we will go over the basic characteristics of the QRS complex and how to measure it accurately.
The QRS complex is composed of three distinct waves: the Q wave, the R wave, and the S wave.
The Q wave is the first downward negative deflection after the P wave, followed by the R wave as the upward deflection after the Q wave, and then the S wave which is the downward deflection following the R wave. The S wave goes back to the isoelectric line or the baseline at an important ending point called the J point. The J point is the intersection of the end of the QRS complex with the beginning of the ST segment.
The J point is an important point that is analyzed in myocardial infarctions and ischemias, as shown in this illustration.
Normal variations of the QRS complex include differences in amplitude, duration, and morphology that are considered to be within the normal range for a healthy heart. The morphology of the QRS complex can vary depending on the lead placement and the orientation of the heart in the chest.
