If you're securely fixing the plywood to the floorboards directly, if the floorboards are undulated will the plywood also be out of level? and if so what do you do to fix this? just screed?
Some have suggested having a thin layer of glue / foam between the plywood and floorboards to help with this, is that necessary in your experience or not?

Thank you for your query and should the floor still show clear undulation after the plywood is fixed, the common method of preparation would be to apply a levelling screed onto the plywood.
