Acute Respiratory Distress Syndrome (ARDS) - Diagnosis, Management, And Major Trials To Date

- Acute Respiratory Distress Syndrome (ARDS) affects critically ill patients and has a very high mortality rate. The management of ARDS is critical with a number of landmark trials to help guide that management.
We talk about the Berlin Criteria used to diagnose ARDS including how to gauge ARDS severity. We then dive into the general management approach including tidal volume, plateau pressures, respiratory rate, oxygen saturation goals, pH goals, and more.
Finally, we dive into the different landmark trials as a way to understand the management of ARDS. This includes the ALVEOLI Trial on low versus high PEEP, PROSEVA Trial on proning, ACURASYS and ROSE Trials on neuromuscular blockage (paralysis), ARDS Network Trials on tidal volumes and plateau pressures, as well as fluid strategies, and the more recent DEXA-ARDS Trial on steroids for ARDS. Our ICU guys went quick to APRV and heavy sedation during CoVid. It's not a physiologic breathing pattern and they felt for patient comfort, ( and easier doc control ), they should all be heavily sedated, so neuromuscular blockade, fentanyl, versed, propofol...they were given more steroid later in the epidemic, proning ( also works best fully sedated and takes no less than 4 nurses to turn them ), and did run them dry. Kept their Ph about 25 and Th about 12 seconds. Would gauge Pl by Pco2 and Tl at 0.6 sec. Tracheostomy at 2-3 weeks. I would guess mortality at > 80%. Thank God for Omicron.

Thanks for checking out the video and for commenting! We use IBW (ideal body weight) as that is the terminology used at our center. With that being said, PBW (predicted body weight) is perfectly reasonable verbiage to use as well. For all intents and purposes, these two terms are used interchangeably in this context and simply base “weight” off the patient’s height. Hope that helps!
