I Herniated my disk and fixed by just working out, stretching, rolling back and getting a posture correcting brace. No surgery to fix issue is definitely possible
I recommend an inversion table for home use between office visits. There's nothing like stretching out that spine for instant relief and they are very inexpensive.

He has back pain due to being obese, and having poor physical fitness, and probably maintained by his beliefs about his back. The fact that he is seeing a chiropractor to 'fix' him exemplifies his poor understanding, and an external locus of control regarding his health.
Vehicle wrecks, accidents at work or home, etc. Don't ever be foolish and automatically damn a person because they have weight issues. More times than not, it wasn't the weight that caused the problem in the first place, but the trauma of whatever caused the injuries to begin with.
