For 'Blood pressure and pulse rate', should we plot eg. 125bpm above or inside of the '120' box? Coz the numbers are now inside a box instead of exactly at a line, or with a range like '120-130' as shown on the NEWS chart. I'm a bit confused! Thanks!!

Hi, what if their vitals are out of range i.e. Low BP, there is no yellow/red zone as per NEWS2 - how is it escalated on a GCS chart?
You will verbalise that you will escalate to medical team.
Should we plot a (x) or a dot (as aligned with the dots being plotted in GCS) for the temperature?
Both are okay as long as in correct box
