We didn't use one but I am not sure if others would recommend or not?

Yes, you need a vapor barrier. This stops moisture from wicking into the structure thru the concrete.
In every jurisdiction where I've been in Canada, the rebar must be tied down to the others with a significant overlap of approx 12 inches
