Investigate the Council and their employees regarding the 'disappeared' file. This is a travesty of justice and the files' 'disappearance' appears to be a serious Crime.
Files missing, phones missing, clothes missing. When do we stop saying it was a failure and start saying it was blatant corruption? Don't forget that a report was deleted from a computer in council offices. Only an insider could have got to that file.

A Muslim SA'd me and all the evidence went "missing" at the police station but I kept a backup and sent them it again..... Then just 5 minutes before the trial started I found out my barrister didnt have all the evidence and realized the evidence gone "missing" AGAIN..... So I gave her the evidence (I had the copy with me) so she got it admitted as evidence with only minutes to spare before the trial started.
There should be an independent investigation into evidence handling.

Government positions or third party employers are tracked in every move they make. Whatever file was being looked at should be able to link back to whoever was accessing systems at that time.
