Gragas assumes Rene has W level 3 and is less likely to try to freeze in his face, but if Rene had taken W level 3 and used it on the wave, Gragas could hold a freeze in his face.
He doesn't level it right away waiting to see if he gets ganked or grag engages, when neither happens he levels q for more healing and clear the remaining minions faster.
