In 99213 time is 20 minutes must be mer or exceeded. In given document(your example) have mentioned 45 minutes, 20 minutes exceeded means 45 minutes also be included right then why don't we use 99213 to satisfy both mdm and time?
Hi , if MDM and time both are mentioned you have to code according to the highest one. If the time is 45 minutes and MDM is low, we have to code according to time because time is high.
And also, when we already have a code 99215, for 40 minutes must be met or exceeded . Why we would use 99213?

Mam e&m how to identify type of question if mdm, and time is not given
If MDM is not given, we have to find out using MDM table.
mam i have a confusion, ques 1,we selected 99215 with 45 minutes, so i want to confirm that how mdm can be compared with time coz time is number and mdm is like low high so how can we code them? or we always have to consider time value?
If time is high and MDM is low we have to code for the highest one, that is 'time' & vice versa. Also if MDM is not mentioned we have to determine MDM using MDM table.

Metastatic, metastasis - which is a primary and secondary. Please clarify
Metastasis involves the spread of cancer cells from the primary tumor to surrounding tissues and to distant organs.
Where as, Metastatic "or secondary" tumors are those that 'have spread' to other parts of body from the original tumor site.
