Why not name a bunch of villagers after you and your friends, leave them in a village that you've filled with death-traps, and then have each of your friends commentate for each villager as though they're playing? Whichever villager lasts the longest wins the game - it'll be just like a life series!

Love this idea, but how about you have the NPCs moving around, maybe wearing different outfits. For a challenge, all the NPCs will attack if provocked and they have to work out which one is you with some suttle clues about either where you are hanging our or the skin you're wearing. Friends will have to work out who you are, and if wrong, have to win a fight with the NPC (could always hide kit around they need to find they can use).
Also, what's the plugin you used for the NPCs?

Get your freinds to make a build of an item (like build it block for block not just said block) in minecraft which you might have in your house and you have to find said object in under 30 seconds. (Effectively supermarket sweep)
I know its late for top comment but I'll repeat it on the next one.
Play a game called Level Out. Have a big field of sugar cane, roughly 100-120, and have them try to keep it all 1 block high at the same time, maybe give them a 10min. overall time limit to keep it level for as long as possible. Every time one of the canes grows to 2 the level timer stops until its all level again. The longest leveled time out of 10mins. wins.
I can see why you don't follow your viewer's suggestions exactly. They try to be funny and add a twist to the suggestion instead of just having a genuine good suggestion. Like they don't want to see the video, they want to see the reaction to the twist.
You should do a video where theres a secret word, but your friends don't know that there is but when they say it they explode
