Project Protect: Undetectable = Untransmittable

  • "Project Protect" is a video series designed to give you information you need to make decisions about your sexual health. In this episode, Adam Weaver. Prevention Program Manager at LAS, talks about the word, "undetectable."
    A person living with HIV is considered to have an 'undetectable' viral load when antiretroviral treatment has brought the level of virus in their body to such low levels that it cannot be detected in blood tests.
    According to the CDC:
    "Scientific advances have shown that antiretroviral therapy (ART) preserves the health of people living with HIV. We also have strong evidence of the prevention effectiveness of ART. When ART results in viral suppression, defined as less than 200 copies/ml or undetectable levels, it prevents sexual HIV transmission. Across three different studies, including thousands of couples and many thousand acts of sex without a condom or pre-exposure prophylaxis (PrEP), no HIV transmissions to an HIV-negative partner were observed when the HIV-positive person was virally suppressed. This means that people who take ART daily as prescribed and achieve and maintain an undetectable viral load have effectively no risk of sexually transmitting the virus to an HIV-negative partner."
    If medication is stopped, the viral load will go back up and the virus will be detectable.

    My viral load is 42 and my cd4 is 940 I'm so happy because it's has been a hard journey ever since I was born

    I have 20 copies and the doctor said I should come to 0 copies to be undetectable and have children but you ( you said you became undetectable when you pass 200 copies to 0 copies I am a little bet confused

    It is still vital that someone who is positive discloses their status to any sexual partners. It’s a legal requirement. Negative people have a right to make decisions and to stay negative.

    Does non reactive mean undetectable?

      Hi, Melissah! No, non-reactive does not mean undetectable. Undetectable and non-reactive are two different things. The specific language for test results depends on the test being administered, but a non-reactive HIV test would indicate that the test is not picking up HIV antibodies or antigens (depending on the testing generation) and so it would effectively be the same as saying the test is negative. Undetectable is used to describe the viral load of someone who is HIV+, they still have the virus, but have used effective medication to lower the amount of HIV in their bodies to an undetectable level. Undetectable individuals are still positive, but their risk of transmitting HIV is effectively zero.

      no ... non reactive is a negative test which mean you dont have the virus ... undetectable means you have the virus but defined as less than 200 copies/ml in your blood

    Undetectable is not the same as negative. Therefore it is entirely possible to transmit HIV while being ‘classified’ as undetectable. And remember this ‘classification’ is not accurate as viral load is only tested every three months.
    It would be better to change this from ‘undetectable’ to ‘according to the test used and it’s limit of detection at the last test it was not able to detect virus in the body, however, there is still virus in the body and during a three month period virus levels can change dramatically’
    Until we have a home testing meter where you can detect the presence of virus that day, it’s not really a very reliable statement to say ‘undetectable’
    The other thing to consider here is that there is no such thing as ‘undetectable’ because the person is still positive and a test to see if they are HIV positive would detect the virus because they are positive, so it is not undetectable, it is just that the particular test used only goes down so far in detecting small numbers of virus.
    One test registers ‘undetectable’ and another test registers ‘detectable’ all of the big pharma companies have a test and the results all differ.
    The risk is very low, but it is still best to use condoms to be extra safe.

      And there is no way it can spike from lets 40 to 1000 copies in a month its impossible .If you the medicine is working on mother to child transmission it can work on anything why because its the same medicine .i have had two children while positive the first one i wasnt on medication yet i only got the neverapine tablet when i went inti labour and my baby is negative
