Surgery: Third year rotations

  • I’m student Dr. Thompson, I’m a third year medical student and In this video I will explain what my surgery rotation was like and at the end give you the Must have resources for doing well on the shelf exam.
    So unlike many students who have 6 and even 8 week rotations in surgery, I was only given 4 weeks. Which is kind of sad because I REALLY liked my surgery rotation. Fortunately it was enough time to study and do quite well on the shelf exam.
    Surgery rotation
    In the OR, students are mainly there to observe. I was invited to scrub in 5-8 times per day. Scrubbing in is when you Don the surgical Cap, Gown, and custom fit gloves and get to be one with the sterile environment that surrounds the operating table. Otherwise you might observe from where the anesthesiologist is working or from the back corner of the OR.
    I drove the camera. During a laparoscopic procedure the surgeon may have both hands occupied with other tools, students often get to “drive” the camera or basically point it at the site your working at.
    I held retractors to widen the incision site allowing the doctor to work intra abdominally.
    I also sutured laparoscopic port sites closed with a suturing technique used to bury the knot, I put a vid on my playlist for you guys.
    Study the surgeries the night before. Know the anatomy as many pimping questions come from properly identifying structures. Know anatomy as it might look from a particular surgery, things look differently laparoscopically than they do in an open procedure, nothing looks like Netters. This is where Surgical Recall Comes in handy.
    Return the next day and report what you have learned. This is true for any rotation, but you want your attending to know that you looked up what you missed and he will likely not remember what it was. This is your time to shine by being prepared to go further than he himself might recall right then. Impress him! So be proactive.
    Do as much as you are allowed to do. Help with every little thing. Get to know your boundaries of this right away. I wasn’t allowed to write notes (dumb I know) but most of you will so learn how to do this day 1 and volunteer to do it every time. Especially post op notes. Learn how to scrub in quickly, efficiently, and before the surgeon so you are ready to go right when the surgery starts.
    The Black note book. Doctors like to see that you are engaged, one way is to write down what they say in front of them, I do this with a small notebook that fits in the pocket of my scrubs.

    Surgical Recall is excellent for preparing for actual surgeries and has lists of commonly asked questions doctors like to pimp you on. Very helpful. It goes through each aspect of surgery as well as just about every surgery you might possibly see performed by a general surgeon.

    Surgery was the only thing I excelled in during my 3rd and 4th year rounds
    As a result, I'm now a neurosurgical resident and I love every minute of it

      Can I ask you more of a personal question? How do you balance your life as a resident neurosurgeon and your personal? With a partner, or a kid, or even none?

      Chris Mentalist Hey Chris, sorry for the late response.
      In terms of that, you'll meet many surgeons who will say it is quite difficult to balance your social life and your job and for my residency especially there will be times when i don't see the outside of the hospital weeks at a time. My boyfriend, of 10 years (we're both 26) and I are both surgical residents so he understands the time constraints and physical hardships residency can bestow on someone. My parents are both retired surgeons so they are pretty understanding as well. Of course, not everyone has such a strong support system that I was blessed to have, however, the amount of hours we work increases the likelihood of burnout as well. Obviously, I have days where I have no work at all, but I'm someone who loves to constantly be on their feet so I hate off-days unless I have plans with family/ friends prior to that. I don't plan on having children anytime soon so that's pretty much out of the picture for me at this point.
      In general, my field of medicine is slightly less predictable than most other fields, but then again I'm not an ER doc either.
      As med students, we're always told that our patient comes before anything else and that philosophy continues well into your career as a doctor.

      Jimmy McManus It's quite hard. In med school, you have to be on your game almost all the time, be passionate in research and advancement of neuroscience research. You have to keep updating on your surgical skills, and by that, even if you do have superior eye-hand coordination does not guarantee you'll be a great surgeon. You have to understand the intricate structures and functional neuroanatomy of the brain. Most neurosurgeons also have quick reflexes and high amounts of patience while standing in the OR or for dealing with the patient's family if something goes haywire. Easier said than done, but as Michelangelo once said, I'm still learning.

    I loved the video and I almost could picture you at Greys Anatomy looool really cool! I can't wait for my rotation starts, and the surgical time is the one that makes me more excited. But I am still in the middle of the second year, and in Brazil it's six years the medical school. In four years we have classes, and than in the last two we are called "interns" and we don't have classes at college anymore, we just stay in the hospitals. So in this time, in the last two years of six, it's when we do our rotation. Really nice to learn from your experience here. Please keep making this videos and good luck with your tests!

      Khaled Delahk Thanks for the feedback. There are some strong opinions for and against off shore medical schools. I think from your point of view, you should treat Ross like a back up. I'm sure you've heard that, but the caribbean schools have additional challenges when it comes to residency back in the states that you wouldn't face at an on shore MD or DO school. But I rotated with a surgeon who makes well above the average surgeon wages and he finished school in the caribbean less than ten years ago.

    Thanks for the video student Dr. Thompson! Do you think it matters what college one attends as an undergrad as far as getting into medical school? For example do you think it would be an advantage to attend a large and well know school rather than a small satellite school?

    If you get corrected in a procedure or during a stitch it’s really no big deal. You aren’t expected to be perfect. He kinda makes it seem like it’s the worst thing possible

    What departments of a hospital should a volunteer experience? At my volunteer orientation we get to select a couple departments that we personally are interested in and one of the two that I chose was the Intensive Care Unit. Do you have any suggestions/advice

    Do you have any advice for premed students who does not want to do research? Most medical schools recommend it and most of my friends have done research but I have no interest in it. I fear that this will be a disadvantage when it's time to actually apply to medical school. HELP !!!

      My solution was to do a semester of undergraduate lab work for credit. It's less demanding than running your own study, and gives you some much needed perspective on the scientific process.

    My hands get shaky whenever i do something that is scary... Will it go away? If it's not.. Can i still be a surgeon?

      +Eldy Eva Scary just means new in most contexts... Keep forcing yourself to get out of your comfort zone, and you will be more confident in stressful, or new situations. Shaky hands will go away with the confidence.

    I'm a bit confused, is surgery rotations last only 4 weeks throughout the whole med school in the us? I mean, itz 12-15 weeks here in Egypt!

      I'm not in med school but I believe thats because rotations only happen in your third and fourth years since first and second are more for learning..

    Quick question. In another video you said( correct me if I'm wrong) "I'm a husband,farther , and then student" I'm just curious why did you put ur wife before you kids ?

      Justin Hunte presumably because if he put his kids before his wife someone would ask him why... or maybe he ranked those two things together but given the nature of English syntax one had to be before the other in the sentence unless he coins a neologism, e.g. Husather.
