Put it this way. In cannon AllMight go his injuries from AFO when he let his guard down for a second. When he noticed what was about to happen, it was a fraction of a second to late to stop it. Point is no matter how powerful someone is, one unprepared moment could change everything.

Why I find him losing to nomu at usj to be completely bs considering the power he holds at that point I do overall like the story even if I do feel at times that it builds stuff up only to backpedal and change direction a good example is with Ibarra there was no real need for that other then to backpedal and make us feel bad for him and his already shitty hand in life I do want a part 2 but I hope that the story will actually pick a direction in the future not backpedaling or relying on shock value.
