actually, Aya doesn't turn her patients into dolls like her father, those dolls are most likely actual dolls (in the remastered version, she hugs a doll, saying to herself that she promised Dio that she wouldn't forget the people that her father killed). instead, she most likely kills patients to relieve them of their pain and uses medicine to make their deaths painless...which is still twisted, but better than how her father was, so I guess she's still not a good doctor, but a better one.

After these conversations with and about the red eyed "salesmen" in this video I'm pretty sure he was the one who placed the book outside. The book Aya finds after her escape with Maria. How did it get there otherwise? The red eyed salesmen can teleport like a ghost and enjoys chaos so it would make sense. After all the book is probably what brought her tendency to become like her father out. It was probably a book full of her father's work, about his "beauty philosophy" and probably about how to turn people into dolls too. The salesmen could be the devil. A lot of classics portray the devil as sketchy salesmen after all.
You can find the book in the playthrough in one of the rooms i think its called "new text on antomy" or something like that. Aya says her father always read it and refuses to look in it because it has "gross pictures"(probably of experiments or such)
What I'd like to think is that Ogre-man somehow switched the real Aya with a clone when Aya found the book. She wasn't seen by anyone and was a bit delayed in meeting up with Maria who went ahead. Also, in the All Gems extra scene, you could see a more grown up version of Aya( I think) with longer hair, which is a lot different from the one we see in the true ending, that one was a clone while the real one grew up(?) inside that glass tube thing. This might also explain Aya's immediate liking towards her father's research at the end of the game because it's not her. Side note: If there's a bigger story to this, I don't know it so... this theory might be wrong

I like to imagine that after Aya becomes a doctor, Dio has already been growing older like her. So, as he said himself, he would be taking a different path. Maybe he would eventually end up on the road to Aya’s clinic. Interested by the idea of maybe getting his eye problem (or lack thereof) checked out. Little did he know that he would run into the same young girl he kissed the forehead of. Due to her promise to never forget the people who were sacrificed, she remembers him and excitedly tells Maria who must be quite surprised. But Dio has also remembered something from that night. What the doll had said. “If.” So he asks to stay a night to catch up and discuss what Aya and Maria have been up to. Getting little bits of info, here and there, he decides to wait until they fall asleep and search for potential clues. He would eventually mind Maria’s diary and find out the truth of her plans. But Maria would wake up and notice he is absent from where he was supposed to be sleeping. Then the rest of the time he is there, he just tries to avoid Maria and her attempts to keep him quiet or dispose of him while trying to save Aya from becoming her father. Cue the title sequence of the sequel.

I am SO tempted to write a sequel of Dio saving Aya ;;; AAAAA (It's possible for me to write tbh- I can post it up on Wattpad but I can't be sure of how this will end out ;; unless you guys are supportive of this idea?)
I know this game is from a long time ago, but I wish they make another game with this. I love everything about this game with the stories, how the characters are really are, and the foreshadowing they brought with this. It may not happen or never will, but if there were a sequel to this I’ll be willing glad to see what happens in the story.

I know this game is from a long time ago, but I wish they make another game with this.
I love everything about this game with the stories, how the characters are really are, and the foreshadowing they brought with this.
It may not happen or never will, but if there were a sequel to this I’ll be willing glad to see what happens in the story.
