This little girl, for example, was SA’d. The dark creature is basically a manifestation of their traumas and are what they’re telling themselves / what they have been told by adults, e. g. she should blame herself; her innocent inner child has been taken, etc. And no worries Rose, in.p my experience, the Asylum part is the most effed up in the game (very much like Alice, haha xD). It’s still messed up down the line, but in my opinion more confusing-messed up instead of gruesome-messed up.
