My dad worked for the FAA for over 30 years prior to his death, there were lots of cuts, issues with people being overworked etc, its only gotten worse. Its a job that requires SO much attention, and with the cut backs people are getting overworked. This was preventable and I am so devastated for everyone involved.

Female pilot here: my suspicion is that 3 things were at play.
    1. The altimeter (a device to allow for barametric pressure) on the helicoptor was either not set correctly or was defective. An incorrect setting gives a false reading on altitude.
    2. The helicopter pilots were wearing night vision goggles in a brightly-lit area. The pilots thought they had the AA aircraft in sight. But, they most likely saw the plane behind the AA flight.
    3. The ATC should have directed the helicopter pilots to hold their position instead of asking them if they had the AA plane in sight. But, the pilots responded that they had the aircraft in sight. So, ATC took them at their word, since the pilots took responsibility by responding, "traffic in sight." The air traffic controller was also overworked since the other one went home early.
Grear reading! I got the same result in my reading as well.

Wasn't the helicopter also not in the proper airspace? Like helicopters are never allowed above 200m and they were at 400m? Pretty sure I read that somewhere along the way and haven't really gotten a clear explanation as to why that may have been.

I could see this. I don’t have any aviation experience and when I heard about the ATC asking that question, it sounded like a useless thing to ask. Like asking someone “hey, do you see that lion running towards you?” Instead of immediately giving them directions to stay out of harm’s way.

On point, Ant. There should have been 1 air traffic controller tracking planes and 1 air traffic controller tracking helicopters. At the time of the crash, there was only ONE air traffic controller tracking both planes and helicopters together. If I'm not mistaken, the FAA had also cut staff recently. So, you're also on point about air traffic controllers being overwhelmed and not getting the assignment they need.

    • @Stronzetty

Tower controllers deal with all traffic, it doesn't matter if they are helicopters or planes. DC has a visual approach procedure, instead of instrumental (which is becoming the norm in most airports and deemed the safest). It is a matter of procedures not working when visibility is low, like at night. I have heard colleagues in the field say, the helicopter had the plane in sight, but it was the one behind.

Federal Aviation Administration officials have been aware of the staffing issues for years. The FAA hasn't been meeting its hiring goals for a very long time. There's roughly a 50% washout rate for air traffic controllers. They flunk out at the academy or flunk out in the field. Capitol Hill, as well as airline executives, have sounded the alarm in recent years about recruitment efforts falling short. The agency has a massive personnel deficit and is about 3000 controllers short.

6:30 I have heard on the news air traffic control had 2? People and went down to 1 person managing multiple flights/giving communication. When it is a job done by two people to level the work load.
    Source: bbc news article
    9:22
Duffy said the US has a chronic understaffing problem in air traffic control.
US media has reported that more than 90% of air traffic control facilities in the country are operating below FAA recommended staffing levels.
"We haven't had enough air traffic controllers in America for a very long time," Duffy said, adding "they are stressed out. They're tapped out. They're overworked. That's no excuse. It's just a reality of what we have in the system."
    6:00 8:30
    Weather issue / breaking protocal
Duffy also questioned the timing of its flight.
"Why would they fly a mission at nine o'clock at night through really busy air space... as opposed to flying that mission at one o'clock in the morning, when there's very little traffic"? he asked.
"I want our military to be trained up and ready to go, but I also want air travellers to be safe as well, and there's a time and a place to do it, not at nine o'clock at night when there's heavy traffic

Most companies have that. One person does a two-three person job. I don't necessarily think it's understaffing as much as cost-cutting (in this case though it's probably both). Two people doing that is expensive. Kinda goes with what he's saying at 7:19. I don't know if Regan airport workers are unionised, but either way in the modern work world that is VERY common.
It makes no sense, because so many people are out of work.

A single person should not being doing the work of 2-3 people unless they are being paid 2-3x the salary and even then, in cases like this, its extremely dangerous. People can literally die in the areas of transportation when it comes to understaffing and cost-cutting initiatives. Do you really want one air traffic controller to be handling 5-10 flights at the same time? That is just asking for a major tragedy to happen.

I'm just very angry about this. People constantly keep saying this is the safest way to travel, but then we have vital, lifesaving positions that are terribly understaffed, employees litterally being underpaid and overworked to the point of pilots and flight attendants facing illness and homelessness in some cases. We have civilians who got shafted, injured, or killed not being able to get any kind of justice, the military doing what they always do, and people are still over here being like, Oh. It was just an act of God. Nothing to see here. Move along. In my opinion, this was a case of extreme carelessness and neglegence. I'm not saying anyone was murdered or anything, but people are too careless with other people's lives and I'm so tired of this. At the very least, this industry is exploitative in a bad way, and I don't know if I can continue supporting it unless things change. It's sad because I love to fly. I want to see everyone associated with the industry in a better position, but these accidents just keep occuring. This time, civilians died and they had nothing to do with what was going on internally at a governmental level or in the airline itself. Nothing can give them any kind of justice at this point. People are blaming trump, but all of this goes back decades. Trump is just a little man. I fear people will hyperfocus on him and nothing will be done to prevent this from happening as much in the future.

It's been upsetting to see people jump to conspiracy theories. Looking at the statistics of crashes/accidents, there's almost 3 to 4 accidents a month! This one just happened to be more public where many people witnessed it, but most of these accidents happen in the ocean/over bases where they practice & never make the news like this. I completely agree with what you picked up, the military does have an issue with some of the soldiers being overly confident in their abilities & that's probably what happened here. Never underestimate ego & mediocrity.

Research a little harder: In 2013, during Obama's administration, there was a $600 million budget cut done to FAA (it's on their website .gov and whitehouse.gov). In 2024, Biden's administration didn't include a budget increase for FAA. Trump didn't have anything to do with the budget, but he is looking into their hiring practice. Both parties are to blame for this mess. No one's hands are clean.

As a lay person, it appears the incident was a tragic accident. Perhaps the DCS is reaching a tipping point because of the increased traffic in an already congested area. I do not think there was a conspiracy at all.
For me, if there is any fault then it is with the helicopter pilot. I have listened to many of the flight recordings, and the helicopter pilot did confirm he had “visual separation.” Which means that pilot assumed separation responsibility flat out. The helicopter was also above the 200 feet ceiling as it travelled along that narrow corridor. Human error.
With that being said, the ATC should have confirmed with the helicopter pilot that he had visual separation with the immediate danger which was the 330 plane. We can all learn from this to keep people safe. Enough with the stupid mud slinging. Fix the problem first before fixing the blame….

He said with the heli pilot only having 500 hours of experience, they weren't experienced enough to have visual separation at night and flying around an airport! The heli pilot overestimated their skill level, and asked for visual separation. Should never had asked or been granted visual separation. The Blackhawk WAS confirmed by CNN as flying too high (they were supposed to be at 200 ft and they were at 325ft. Plane was at 300ft).
The big thing: a lot of pilots are saying if you see a plane and it doesn’t look like it’s moving, you are on a collision course.
They also said they had one guy at ATC doing the job of 2 people.
Blackhawk kept saying they would maintain visual distance, so…

There has been six suicides in November and December of last year with air traffic controllers. They are extremely overworked. They’re only allowed to work two hours straight and then they need a break and then they can go back to work but only for two hours maximum at a time because of the high intensity of the job. I know that air traffic controller that was responsible for that route, is feeling the weight of the situation and the guilt, even though the helicopter pilot said that they’re asking for a visual separation, which means the air traffic controller is no longer responsible and that the helicopter pilots are taking responsibility for getting in line behind the plane to land, 30 seconds later they ran into the plane.
That whole area is very constricted and you don’t have a lot of space to fly because of the White House. You’re not allowed to fly over the White House as a commercial plane. You have to follow the river in order to land, and the helicopters are only allowed to fly over the White House if they’re picking up somebody so they’re already given a very limited space but yeah I agree. I think there was some laziness and arrogance in the helicopter pilots and the air traffic controllers.

This happens a lot with cheap companies and gov pushing too much on emphasis on less is more but they forget everything matters in order to be called a professional in your career fields
