There's no mention of having to use the blood that she gives you in order to progress and receive the rune. You have to use the blood somewhere other than in the hunters dream and then go back to her until she turns into a head. Then you kill her, take the brain fluid, warp back to the dream, head back to her and give her the brain fluid and get the rune.
so if you have both brain fluids in your inv when you talk to her the first time she takes both and you can't go any farther? cus I had both talked to her once and now both are gone. and of course no more will drop.
On her final shape (when she’s on ground & asking you for the last time to give her fluid) hit her she will drop one fluid then she will come again to live give her the fluid and she will give you the milkweed 👍🏿

Theres a lot of things blood borne didnt explain, i was extremely confused first time playong it doesnt tell u to do anything it just gives you hints that can easily be missed with boring dialougs
I can confirm that the vial is not the factor. I have both Adella's and Iosefka's vials in my inventory and I can give her the 2nd brain fluid. You just have to keep coming back until the conversation changes. The game just wants to piss you off.

I don't know if you finished the questline already since this comment is 9 months old by now, but I think you might have to use bloodvials you got from the characters such as the nun with the braid, Arianna I think she was called or something, or the other blood you can get from the blonde lady. Having Iosefkas blood didn't stop me from completing this the questline but the other npcs bloods did untill I used them. Best of luck! :]
just did this if you give her first one cool but the second one cant be given to her without refreshing a few time...i died a couple times but i think you can just teleport to dream and back a few times.
You might need to go to the Hunter's dram a couple of times in between to get the items (Blood of Adeline and Balcony Key) when giving her the first two items. The third time once she turns into a blob in the corner you kill her. People are also saying not to have any special blood vials in inventory, but I'm not 100% sure on that and never needed to do anything myself.
Normal Hack I found the guide helpful, I was able to complete it. Had to go back n forth from hunters dream to get her to progress to the next point in the quest.
Easy way: get the first brain fluid and give it to her, kill living failures and lady maria, get the second brain fluid, give it to her, restart the game, go to her, hit it, and then she'll give you the third. That's all.

Awful guide 'slash her in the corner of the room' do that and u kill her thus failing it.DONT DO THAT,after the second brain fluid,keep going in and out of the hunters dream until she is out of the chair,then when u find her in the different side of the room kill her to get the third fluid,then go back to the hunter's dream and back again and she is respawned,only then u can give her the final fluid
+tassssssssss I accidentally killed Adeline after giving the first brain fluid. I went back to the Hunter's Dream and back to the Research Hall and she respawned back. Extremely rare to see an NPC respawn in exception to the Doll. Oh, after Adeline turns into a blob after you attacked her, there's no need to go back to Hunter's Dream. Just wait until she goes back to blob form again after squishing (attacking) her and she'll be ready to talk to you again. One more thing, wear Lady Maria's set when you talk to Adeline to get an extra dialogue. Thank you for the guides :D.
+Big Ricky Did you try to reload your game? I remember turning off my PS4 when I gave up on doing her "quest line" when I messed up and was sad :(. I went at it the next day and she was surprisingly there. Try Go back to Hunter's Dream and quit the game and load your save again.

For me she only took one then I had to inject her blood 3 times and go back to Hunters Dream a few times then I could give her the 2nd

You should only have to hit it once and it’ll give you the item
You probably already have 2 brain fluids. Use the blood she has given you and you can give her the fluid

It's because you have blood from either Iosefka or Adella
You can only have 1 vial of blood from an NPC use that vial and then talk to her again.

Kos parasite weapon should be the strongest because its the hardest weapon to obtain but its probably one of the worst
I don’t think you understand how to use it then because it’s incredible. You probably have a skill/bloodtinge or skill/strength build
