I’m pretty sure the reason you don’t take damage from the curse room door is that you’re going fast enough for your hit box to skip over the pixels that do damage. (I have no proof of this is just my guess).
To expand on what I mean, i believe that when the game updates his hitbox location, it sees that it never interlaps the pixels on which you take damage from the curse room. This can happen, as if your fast enough, your hitbox could be just infront of the pixels when it checks for this, and on the next frame it checks, be passed the pixels, meaning the game never sees the hitboxes overlapping. Once again, I have no proof of this, and have not tested to see if that was the case. I definitely could be completely off.
I had an idea for an item recently. Charred hook. It’s a burnt crochet hook. When you pick it up, you gain a 1.5 times damage multiplier and a 2 damage up. You can only shoot one tear at a time. Upon shooting, you shoot a burnt crochet hook which is connected to you by a strand of yarn. The hook is controllable and will never fall. Upon hitting an enemy, deal your damage and immobilize them. The hook pierces enemies. Hitting an enemy also connects the string to them. When returning the hook to Isaac, all connected enemies take 2 times Isaac’s damage, plus 10 multiplied by enemies attached. Bosses are slowed, and upon removing the hook from the boss it leaves a pin which attaches to the yarn and lets you hit the boss again. It can be pierced only five times and against bosses it deals 2 times Isaac’s damage multiplied by amount of times the boss has been pinned. Description: “Thread the needle.” Appearance: Isaac’s body is wrapped in white yarn with the charred hook stabbed through him. Synergies: Brimstone (Hook is replaced with azazel’s horn) and technology both replace the string with the respective laser after returning the hook to Isaac. Monstro’s lung: Isaac does the string attack up to 7 times. Multishot: string attack repeats the corresponding amount of times. Ive been watching Mattman for 2 years now, and I recently went back and checked out some of his older videos from 2 years ago, and man has he changed. He didn’t yap a whole lot (which I enjoy btw), his pog yell was a bit different, and he was definitely less confident about himself, you can hear it now, that’s he’s just relaxed and talking. At max speed I sometimes go through things in Isaac. As a dev, if I had to say, there's a threshold check on boundaries happenning, and isaac goes fast enough that it "may" skip the whole threshold range, depending on starting position, and clip through a curse room door. There is probably a way to get it consistently

Hey Matt! Here's a little thing I've noticed over my playtime. Whenever you press a button and enemies spawn from it, they will always spawn on all doors in the room, and that includes secret room walls. So if you ever press a button and see enemies near a wall, that's a secret room.

Item Idea! Dionysius: All pedestals are tripled, and increased chance for secret room pedestals and for items to turn into pedestals. If you get hit, even on mantle, the upgraded sword immediately falls and eats through 5 lives. The way to get it would be to use a suicide king card in a room with Damocles in it.

Hey matt! Computer Science student here with the curse rooms you are actually onto something. Now beware that I'm not exactly sure of many variables of the game so what I'm saying here is in no way exact. The binding of Isaac runs on 30 ticks per second. Which means that all the stuff you do in the game gets calculated ever 1/30th of a second so if your game runs on a better fps than that (which I'm sure it does) It's not really a precise representation of the game since not each frame corresponds to each calculation (it would if you ran it at 30fps). Game engines interpolate (simulate) the frames that are in between, so if you walk into a curse room in that exact moment where the frame of you walking in is the interpolated one (is an estimation) you don't take damage. This is fine because normally it wouldn't have much of an effect since it'd be pretty rare for you to do something for less than 1/30th of a second. It's probably happening to you more, because you're using a very high refresh rate with a high fps count and have a maxed out walking speed, which all contribute to a bigger probability of you being in an interpolated frame. There's also an easing curve to the movement, meaning you don't start at the speed 2.0, which would explain that you had to stand somewhere on the skull to walk at full speed -> which increased your probability... I think that should be it I'm not 100% sure though. You were still very lucky that that happened so many times in a row. But no one is surprised by that. I don't think they will fix it though since it's a problem with the design of the whole game loop and that's not something you redo just via a patch. There's other ways they could fix it though! I hope I was helpful

The walking through curse door is probably the same thing as minecraft's walking over gaps. Isaac runs at 30 ticks per second (I think), so as long as you move in such a configuration that the distance traveled in 1 tick covers the entire spike of the spiked door, you dont get damaged butheywhatdoiknowimjusttheorising
15:47 Proctosis can be pseudo counteracted with lump of coal iirc, proctosis reduces damage over time and lump of coal increases over time, so they cancel out (with a gradual increase in damage due to lump of coal being stronger), but its been years so i dunno

ur probably right about it being based on distance for the damageless curse room. im guessing having high speed means u can skip over the damage trigger in one frame, as long as you're lined up correctly.
The curse room thing I’m pretty sure is probably because you’re going so fast you skip past the frame where your hitbox is in the damage hitbox and instantly go through the door instead.
i had another fun idea for an item. what if there was like an item that made it so that Food items become slightly rarer, but they give you all stats ups whenever you pick them up
Perhaps something that could be added as an interaction is using either of the infested? pill (the one that causes spiders to spawn from enemies) in a room with infestation (the first one) which would lead the the infestation turning into infestation 2 as it is INFESTED with spiders due to the infest and makes spiders appear from killed enemies.

Hi Matt, recently on day 10 of October my grandfather passed away, he was like a best friend and a father to me. And watching your videos doesn't really cheer me up but I would always like to mention one of your videos when talking to my grandfather when he was in bed, I would pretend you were my actual friend in real life and the stories you tell, I would retell in 3rd person, he always had a good laugh when I told him of your stories and day to day life. the not taking damage thing... i went back and went frame by frame. there is a certain distance per frame, and if that distance is far enough, you ignore damage. so basically its based on how far you go each frame and where u land on the door on each individual frame
Edit: Oops, I just realized you said almost the same. I think I misunderstood your comment initially. Since it's an area in front of the curse room that damages you and not the actual process of entering the room, it seems to me like that's just the usual hitbox detection problem that allows players to pass through stuff in other games too. In this case I assume when the player speed is high enough and you set it up correctly the game doesn't detect you touching the damage area until you are already touching the teleport area and then I guess it just prioritizes the teleport and doesn't damage you.

I don't think they will fix it though since it's a problem with the design of the whole game loop and that's not something you redo just via a patch. There's other ways they could fix it though!

The walking through curse door is probably the same thing as minecraft's walking over gaps.
Isaac runs at 30 ticks per second (I think), so as long as you move in such a configuration that the distance traveled in 1 tick covers the entire spike of the spiked door, you dont get damaged

15:47 Proctosis can be pseudo counteracted with lump of coal iirc, proctosis reduces damage over time and lump of coal increases over time, so they cancel out (with a gradual increase in damage due to lump of coal being stronger), but its been years so i dunno

ur probably right about it being based on distance for the damageless curse room. im guessing having high speed means u can skip over the damage trigger in one frame, as long as you're lined up correctly.
The curse room thing I’m pretty sure is probably because you’re going so fast you skip past the frame where your hitbox is in the damage hitbox and instantly go through the door instead.

Perhaps something that could be added as an interaction is using either of the infested? pill (the one that causes spiders to spawn from enemies) in a room with infestation (the first one) which would lead the the infestation turning into infestation 2 as it is INFESTED with spiders due to the infest and makes spiders appear from killed enemies.

Hi Matt, recently on day 10 of October my grandfather passed away, he was like a best friend and a father to me. And watching your videos doesn't really cheer me up but I would always like to mention one of your videos when talking to my grandfather when he was in bed, I would pretend you were my actual friend in real life and the stories you tell, I would retell in 3rd person, he always had a good laugh when I told him of your stories and day to day life. the not taking damage thing... i went back and went frame by frame. there is a certain distance per frame, and if that distance is far enough, you ignore damage. so basically its based on how far you go each frame and where u land on the door on each individual frame

Since it's an area in front of the curse room that damages you and not the actual process of entering the room, it seems to me like that's just the usual hitbox detection problem that allows players to pass through stuff in other games too. In this case I assume when the player speed is high enough and you set it up correctly the game doesn't detect you touching the damage area until you are already touching the teleport area and then I guess it just prioritizes the teleport and doesn't damage you.
