I think that Italy would join Central Powers during WW1 and would be important in colonial fights. But Austria-Hungary should give them Trieste for exchange (not Tirol since Tirol was basically useless for them). Would it make Central Powers won? I think not, Italy may had strong navy and decent aircraft but had weak land and colonial forces, which would result in the same as they ended WW2
Its not a question of loyal, central powers take the first step against serbia so italy isn't joined with them cause the alliance was defensive. More the Austria and hungary are in bad relationship with italy (example is the disaster of Messina when everyone run to help italy, but some generals of Austria are go to the emperor and say that is a good moment to attack us).
