it is very unusual and not at all correct for someone to use the title SIR with their last name and not include their given or christian name. Protocol is that the Sir would go with Arthur, not with Conan Doyle. No one would call him Sir Conan Doyle. Sir Arthur or Sir Arthur Conan Doyle but never as it is shown on the title page. (note Conan was a middle name not part of his surname, which he adopted himself to give credence…Arthur Doyle is not that impressive. Though his descendants have used it as a double barrel surname. I suspect this title page has been made by an American who doesn’t know the protocol. They love to say Duke too, without adding the name when addressing a duke directly (which totally wrong). If they are introduced to a duke the correct direct address would be Sir out of respect. Same with Prince or even majesty. Americans are clueless.
