If Heather knew she killed Leisel's daughter than maybe she would have some guilt. Because Leisel was nothing but good to Franco she defended him, protect him, and even tried to get justice for his death.

It wasn't a deep cut. Also, you tend to not feel all of the cuts, scrapes, etc DURING a fight. You really feel them afterwards. And it was the poison that Heather put on the hook that killed her victims, not the stab wounds themselves.
I think it’s also a reference to how Empress Sisi died. In the late 1800s she was attacked on a bridge by pedestrian wielding a shank, he seemingly punched her in the chest with the blade between his fingers and she seemingly walked away unscathed until she removed the tight corset she was wearing to reveal a deep stab wound that bled out immediately and she died afterwards
GH knows how to keep a villain alive for decades. Heather is the longest running "living" villain. She pre-dates the Cassadines.
