The saddest thing for me is, Rash doesn't know what the infection is. He's a criminal, so he doesn't engage with society or learn what's happening outside. When he got infected, he was probably scared, and confused, not knowing what was going on. But when he saw Willow then, he might've thought there was hope for him. His best friend had come back for him. But after seeing her walk away, he must've been terrified. Everyone had abandoned him, he had been locked in that room for who knows how long. Everyone left him, and when he saw his best friend leave too, that's heartbreaking.
According to the piggy wiki the piggy virus will give loss of control and aggression (also head twitching, pupil expansion, and eye color change.) So rash could see what he's doing but can't control himself, sad.
What I thought what happened was Rash escaped prison before Willow and when finding shelter he got scratched, so he reluctantly locked himself in the room to protect Willow when she came by.
