I think that the situation with the GP can be assed like this ( I am answering this before listening to his questions or her answers) :
First things first, this patient is having heavy periods which cause her alot of pain and distract her from her day to day activities and progressing through her career (the condition is actively disturbing the patient daily)
Secondly, this patient , eventhough she did not finish the treatment course, seemingly can not withstand the pain that she is in. This indicates that either she needs stronger pain killers, and at that point OBGYN should be consulted, or the patient is very impatient. To deal with the second case, the GP should explain to her that a surgery has many risks, like risk of infection, excessive blood loss, having an undiagnosed syndrom that causes her to crash in the OT, the waiting list, or even how her body will react to the scar. If the patient is well aware of all of these risks and still wishes to go on with the surgery, then I think the GP should step down from taking from leading the patient care, and that responsibility to the surgical team in a hospital.
I also have another comment. The patient seems to have come to the GP to inform her that she is going private instead of actually discussing options with the GP. I think that this has both pros and cons. The pros are that this reduces the load on the NHS not just for medical professionals, but also interms of resources from sterlising surgical equipment to the antibiotics prescribed to the patient after the surgery. The con here, however, is that the patient believes that the GP is no longer of use to her. This is critical because it means that the patient will not depend on the GP for other matters as well since the patient feels that the GP is delaying or even objecting giving her the care that she wants. The consequences are that more people go to private care which hands them what they want, and people who can't afford private healthcare don't visit the GP as they have lost faith in this capabilities to treat them.
The forth thing that I want to mention is that the doctor has gotten very defensive. Instead the doctor could have assured the patient for example by showing her that progress was made, even if it was small, and that the treatment is working but it just needs time. Additionally, the doctor could have told the patient about studies for example that show that the medication can have almost a similar effect as the surgery without the risk of the surgery. I think from our experience with GPs, we are not really given scientific data or even a resemblance of it during consultations. I think if patients are aware of how viable and reliable their treatment plans are and have been across multiple candidates, they might just continue to trust our judgement and not rush into thinking that GPs are refusing to help them.
