I had the same problem what I did was I downloaded the wrong update so I went and downloaded the right one then I tried it again and it would not work it told me that it could not find the file. This is what I did to fix my problem Erase everything from your USB drive then on your computer delete all the PlayStation updates that you downloaded next start from scratch create the folders then download the correct firmware and plug it in your ps4 and reinstall the firmware and it should work that's how I got mine working.

Yo the problem is the hard drive bro, I fixed it, you need to replace your hard drive, well thats what I done and it worked. I didnt even buy one, I literally gave up and just thought id try one last thing. I took my old ps3 hard drive and changed it, it worked first time just make sure you have the 850 mb update on the usb not the smaller size one and you should be ok. Any hard drive will work but for now im just gonna use my ps3 one
possible solutions : Put the USB in after you have gotten to safe mode menu. If my USB is in before the unit is turned on, it will not power it up. Make sure USB is FAT32 or exFAT format. Make sure you have the correct file, the partial update is just over 300MB, the full install update is over 900MB. Make sure the directories on USB are spelled with capital letters and no spaces and file name is preserved.

I said let me try the easier of the 2 first. lol to my surprise it actually worked, it didn't recognize the USB first but just came out to the "Safe Mode" options screen and tried option #7 again and it worked. If you do what I do you travel with my system then just know you could risk messing up your hard drive. It'll still spin yes but it's the most sensitive part inside most consoles. You might need a new 2.5 hdd or ssd

get a usb make sure its the right format download the update from PlayStation.com make a folder called PS4 and in that folder another folder called UPDATE and working within that is the update now now on ur ps4 got into safe pick the 7th option plug in you usb and click ok

try this recovery data i think most of the Hdd inside the ps4 with problems like this is corrupted. You need to start from scratch. in my case my ps4 got shut down with the power interruption and when i tried to open it when the power comes back i got the same message. I literally formatted my internal hdd by transferring it to a hdd enclosure then connect it to the computer and formatted. make sure to format it to fat32. I personally had to take out my PS4 hardrive clean the inside of the PS4 as it was very dirty then put my hard drive back in. Once that was done I then used the USB Method to do the final option on the list of options. My PS4 was completely fixed. The only downside was having to reinstall everything but that isn’t that bad.

So I had the safe mode death loop happen. The only option I had was to initialize my ps4. After that all my data was deleted and I didn't back it up. I have my account saved on other ps4s (Friends family etc.) I don't have internet and I used a hotspot to get my account downloaded to my ps4 but my data is not there. Anybody know how to fix this issue or is it impossible??
Tech support should be able to help, as long as you have all you're information they should be able to pull it up. all you're transactions, I'd you used a cc. same thing happened to me once

I've tried multiple times to initilize my PS4 but every time It claims is not the latest update, so it can be used, I've downloaded 5 times already from the official website claiming is the latest one (3.55) , but the PS4 says the same thing, "file can be used, please connect a usb device that has the 3.55 version or higher" I've checked that all the folders and file name match with the instructions and even other USB drives to no avail, even clear out a 2 GB one with just the update file and still nothing, this all happen when I changed the hard drive.
Zach Sparby yeah you need to find the firmware program its like double the size of the regular one, just type in PS4 recent upgrade firmware and you should find it, like I said it's easy to tell apart because its double the size of the one you get from the original site

norbert oleksy wrong format bb just plug it in a computet, open documents and right click on the usb, click on format and then idk i forgot but i know that there is an options section where u can choose the format and there are 3 different types, pick the format that has something like "FAT" in it or something i forgot lol
Two problems might be .Usb port not working .USB not working Or Try cleaning the usb port with compressed air take out the dust Or clean the USB it self or just use a different one. One of these options will work trust me.
Mine finally worked after getting stuck every time I try to Initialize and put me back on safe mode right after. So what I did was just literary changed the hdd for an old one that my friend gave me.

I've had 2 hard drives the latest not even a month old, the system software update has apparently fucked everyone over.. forcing our hand to buy a ps5 it seems but fuck that I'd rather get a gaming pc or a Xbox if this is what sony does.

Yes and no. I originally had a 6TB 3.5" hard drive powered to my PC and connected to the PlayStation. Had to scrap this setup. I purchased a 4TB Seagate 2.5" external drive and opened the enclosure. Unfortunately this doesn't fit into the PS4 but this hard drive works and updated with no problem. Might be compatibility issues with the hard drive or a software issue with the update. The update was out of date by the time I updated again and got the new hard drive. There's a couple variables here because of my setup so I can't pinpoint exactly.
Damn I just got the standard hard drive that comes with the ps4, maybe ill buy a new one and see if that works, I don't know how my shit corrupted in the first place, no power cut interupted the newest update an my p4 was working fine until I tried to play a game Nd it kept saying the disc can't be read an then forcing me to eject the disc, once I switched it off an on that's when I got this loop, no matter what I do I just end up and the same screen. Just to check if it was the hard drive I put my ps3 one in my ps4 and guess what.. it worked first time. I guess my ps4 one was corrupted beyond saving. I'm just gonna use this till i get a new one then back up the data

Guys I think what the problem is the hard drive that’s in the ps4 because I had this problem so I just did a little research and decided to take the old hard drive from my ps3 and insert it into my ps4 to just like test if it would work. And it did work. I was so stressed out about the situation

The harware is faulty if you get this. Best option is to get a new hardrive, download new operating system then trade it in for a new ps4

First attempt I downloaded the wrong file after I noticed that I went back and downloaded the correct one and tried it again but it told me that It could not find the file so what I did next was I erased everything off of my USB then I went on my computer and deleted all of the PlayStation updates that I downloaded next I started from scratch I created the folders then I downloaded the correct firmware then I plugged it in to my ps4 and it worked try it

format your usb to FAT32. Be careful though it will erase the data on your USB.

Okay...couldnt get the PS4 to find the usb drive, even after reformatting the usb. So I went into safe mode and went to "Update System Software" and then chose "Update using internet" aaaaaaand it downloaded, installed, rechecked the system and it fucked me...I think we all need new PS4s.

Solution: open ur ps4 and remove the hard drive, clean it and put it back in. After it will let you initialize

I've had 2 hard drives the latest not even a month old, the system software update has apparently fucked everyone over.. forcing our hand to buy a ps5 it seems but fuck that I'd rather get a gaming pc or a Xbox if this is what sony does.

So I went to my usb drive erased everything then I went to the my computer download files and deleted all the PlayStation updates that I had downloaded next I followed the steps created the folders and downloaded the right file from PlayStation and I plugged it in to my ps4 and it worked

I've had my PlayStation for 2 years and now this whole b******* I've had to reinitialize my Playstation twice reinstall everything and then I still get the same issues
I'm have a similar problem. I get through the whole reinitializing of the software and when the system reboots it displays the screen asking to connect the controller to the USB as if nothing was ever installed.

Yes and no. I originally had a 6TB 3.5" hard drive powered to my PC and connected to the PlayStation. Had to scrap this setup. I purchased a 4TB Seagate 2.5" external drive and opened the enclosure. Unfortunately this doesn't fit into the PS4 but this hard drive works and updated with no problem. Might be compatibility issues with the hard drive or a software issue with the update. The update was out of date by the time I updated again and got the new hard drive. There's a couple variables here because of my setup so I can't pinpoint exactly.
Damn I just got the standard hard drive that comes with the ps4, maybe ill buy a new one and see if that works, I don't know how my shit corrupted in the first place, no power cut interupted the newest update an my p4 was working fine until I tried to play a game Nd it kept saying the disc can't be read an then forcing me to eject the disc, once I switched it off an on that's when I got this loop, no matter what I do I just end up and the same screen. I'll most likely try out a different hardrive and see if that works, fingers crossed it does. Thanks for you help

Just to check if it was the hard drive I put my ps3 one in my ps4 and guess what.. it worked first time. I guess my ps4 one was corrupted beyond saving. I'm just gonna use this till i get a new one then back up the data
The dumb thing is you just have to do this steps if you don't have a USB or have the same problem like this guy Step 1 go on safe mode ( if you wanna go to safe mode you just have to hold the stared bottun Step 2 Press on initialize ps4 (number 6) it says is going to delete your user but it's not going to, but it will delete your videos and picts
Step 3 your ps4 will reset and it will stared over like just when you got your ps4
Step 4 when your in a user just log ind to your psn account and download all your games back of your games are ditgialte you just go on libray and go right to the Bottun where it says buy and download it back

Guys I think what the problem is the hard drive that’s in the ps4 because I had this problem so I just did a little research and decided to take the old hard drive from my ps3 and insert it into my ps4 to just like test if it would work. And it did work. I was so stressed out about the situation
