They are small since many streets in Japan are narrower than the roads you can find in the US. And they also take account the height and stature of a common Japanese. So yes, there's a reason why it is small.
Ambulance service is free in Japan and people call them for everything but they are required to use lights and sirens for each call. No point of reckless driving for a minor call.

why do they drive so slowly, is there a stringent speed law on ambulances in japan? like I get the intersections but sometimes it looks like straight open space and they're still going nearly the same pace as other people on the road.

I guess they drive slowly, because it is a big city with lots of traffic. I mean there are easily 5 or 6 lanes and huge junctions. Going over it very quickly and traffic comes from the side and crash. It actually makes sense for them to drive a bit more carefully, rather than to risk it.

They drive slow at red lights to avoid accidents, like with the truck at 3:22. A European or American ambulance would probably have gone faster, only to be hit by the truck, resulting in more injured, and no good to anyone. I like their caution, it's perfectly sensible and shows good training. After the truck has gone through, the ambulance drive can't see what's behind the van, and edges forward until he can see there is no other killer truck headed for him.
By the time they get there the patient would be dead or near death or in serious critical pain wtf with the speed I mean hell I could get there faster by cutting through short cuts in the city on foot ffs

The wailing siren is actually used by nearly every emergency vehicle. The dual-tone sirens are for smaller vehicles/some ambulances.
In Chicago, they use the same EQ2B sirens heard from most American Fire Trucks.
