No, Thomas knew that Caroline was having his child. He was kept away from his son's birth because his father and step mommy decided to raise Douglas as their own and cut Thomas from Douglas's life. This all stemmed from the night that Caroline and Ridge separated and Caroline stole Pam's pills, went to a hotel, took the pills, Thomas called her, and he went over to where she was. Not realizing that Caroline was already high on pills, she and Thomas were drinking and one thing led to another, Caroline ended up pregnant and Ridge thought the child was his until Caroline told him that she and Thomas spent the night together. So, Thomas knew that Caroline was pregnant with Douglas.

Ridge didn't tell Thomas the truth until a little while after Douglas was born, so in this particular episode, Thomas didn't know that he was Douglas' biological father

I have to disagree with the caption on this video. Every home birth, water birth, and woman handles this process differently. Saying that "tv birth" isn't "real" is only accurate for some women. Just because you birth at home or in the water, doesn't mean you'll have a doula, doesn't mean you'll need or want your husband/partner that close and involved, doesn't mean you won't scream your head off with each contraction. This version of birth was different than many others on tv, but it is not more "real" than other births, not for every woman!
While I do agree with you that each birth is different and each person handles labor differently/has different coping strategies, I have never seen a soap opera treat a birth as anything less than life threateningly dramatic and patently over the top. And frankly there aren't that many water births on TV regardless of genre. Therefore this portrayal as a calm, physiologic, intimate event between the birthing person and her partner is something I've never seen on TV. Additionally she utilizes a number of coping strategies I talk about in class as well as the partners involvement with the comfort measures is another aspect I talk about in class. Overall when you look at the way birth is typically portrayed in the media this is the exact opposite
