Brother you don't need to give your date of birth or mothers name this is magic too. What you need to do is before you go to sleep read Surah Fateha 3 times, last 3 surahs of Quran 3 times( each surah 3 times), ayatollah al kursi 3 times, and last 2 ayas of Surah Baqrah 3 times. After reading all this blow on your hands and Rub all over your body. This process is known as Ruqiya and repeat it everyday and do it whole life I can assure you by the will of Allah SAW you will recover InshaAllah.
