I understand the grind very VERY well and Carl you definitely deserve at LEAST one million TH-cam subscribers already. Your jokes and puns are awesome. The editing is great. Your attitude is awesome. The end segment educational moments add something unique. The world needs more Carl in their life. Doubt you'll see this but I've partially gotten to where I am as a streamer thanks to you. Thank you for defending GMO foods, it is so ridiculous to me that so many people get up in arms about them when they have already done so much good and have the potential to do so much more for us. As with a lot of scientific progress we are held back by a vocal minority of the population that is willfully ignorant and the scaremongers that exploit that minority for their own benefit.
Keep up the great work you do for advancing medicine, I'm sure you know this but the work you do is literally saving lives
We are against gmo foods because there is no long term testing on them - this goes for other things like vaping as well. We are running full speed for cost efficiency and ignoring long term health risks.
Henry Mullin Do we run long term studies every time someone comes out with a new varietal of apple? I don't remember people protesting Honey Crisp apples because they were genetically different from other apples, and there is a much larger genetic difference there than with GMO foods
Snark aside, GMO foods have very tiny changes made very precisely to achieve certain outcomes. They often require fewer pesticides and less fertilizer. They have been used to alleviate nutritional deficiencies (look up golden rice), and can be made to survive and grow in climates where they otherwise couldn't, saving lives all around the world. If we find them to do any harm in the future of course we will have to reevaluate, but as far as we know, and as far as we have any reason to believe, their benefits far outweigh their potential downsides
Personally, I'm against the modern corporate/intellectual property uses of GMOs. Roundup Ready crops keep farmers dependent on buying Monstanto products and encourage overuse of pesticides. We need more things like Golden Rice and less sterilized herbicide-resistant corporate cash crops.

Hey Carl! I was so interested in your science talk (as always). Since you work in that field you might make a guess about how long it will take until gene modification gets a big thing. I'm struggling with excruciating pain that almost nobody can comprehend. It's really bad. As researchers found out its due to a gene disorder.
I'm not sure if this would cure me and my "pain memory" could be erased. But it's very interesting.
Could you talk and share a little bit about pain and pain memory?
I don't want to beg for any likes but as far as I know it's the best way for Carl to see the comment. Maybe you guys can help me
I'm not Carl and I know nothing about "pain memory," but as far as gene therapy goes I can give you a little bit of information. When I was taking biochemistry of disease I was asked to write a paper on how I would cure prostate cancer. One of the big stats of cancer is that there is a DNA-proofreading gene called p53 that is mutated for altered or loss of function in over 90% of all cancers, and part of my proposed solution to the prompt was to restore that gene's functionality so that the body can recognize cancerous cells and tell them to commit to apoptosis (cell-suicide) more effectively. This, like targeted chemotherapy, has the same problems as gene therapy in general:
1--you have to find a way to repair the mutated DNA
2--you have to find a way to get the repairing process inside the defective cells
3--you have to find a way to get the repaired DNA to last after you put it somewhere
4--you have to find a way to get enough of the above to happen that the result is good while minimizing the harm to cells that are NOT defective, so that you don't kill a person.
While trying to research this in 2014 or so for the class, I came across two main methods being considered and tested out as far as, "getting the treatment to enough of the right cells and not to the wrong ones." Those two methods were using viral vectors (as Carl is describing above), and using nanoparticles. Both obviously have pros and cons. I was not able to learn enough in the timeframe given to seriously consider using a viral vector, so I went with nanoparticles. Likewise, I don't know the intimate details of CRISPR, but since any attempt to do gene therapy is going to involve engineered DNA sequences being delivered to the nuclei of cells, you're going to need some kind of vehicle to transport it in to keep it safe from nuclease enzymes that normally munch up extra bits of DNA. Viruses are great because they keep their DNA/RNA & necessary synthesis enzymes enclosed in a capsid (e.g. that icosahedron Carl mentions) to protect it from the body's immune system that would otherwise destroy it. In this way you can safely deliver it to cells where it can reach the nucleus and integrate into the host's own DNA (if it's the kind of virus that does so; Adenoviruses do)--so with a little tinkering you can get it to deliver gene therapy instead of genes that enable the virus to reproduce. The huge achievement with using viruses like this is that you solve problem #3 above--the fix'd DNA is going to become integrated into the host's chromosomes somewhere, and won't get munched up by nucleases that normally clean up extra bits of DNA. This treatment is not a new idea though--it has been around since at least 1998:

...and you're just now seeing stuff start to get approved 20 years later. That's the typical timeline you can expect for new medical treatments. First availability is about 20 years behind the science b/c of how careful scientists are to make sure they don't accidentally kill people, and that's only going to get worse as the populace at large continues to abandon their belief in (and funding of) science, spout conspiracy theory nonsense, turn to alternative medicines...or, I'm going to just say it, vote republican. Unprofessional of me for sure, but hard to dispute right now in my country.
Nanoparticles are kind of like jerry-rigged viruses in the sense that, like a virus they are a container that you can get to deliver a payload to a cell. Unlike viruses, they can deliver a drug in addition to or instead of an additional gene. Like viruses, they need to exploit a receptor that the target cell has on its surface in order to get inside, but while viruses are usually very specific for which receptors those are, you can hypothetically choose whichever one you want for use with a nanoparticle. Unlike viruses though, a nanoparticle's payload isn't as guaranteed to make it to the nucleus once inside the target cell, and nanoparticles themselves are more likely to be toxic in general to the person being treated--they are often made out of metals (gold, zinc, etc.) or silica (this also makes them more expensive to use, although there isn't a lot of mass there overall so the bulk of the cost is production method, not raw materials). Also, if you use this method, you have additional challenges with respect to problem #3 above--if you're just throwing a DNA sequence into someone's nucleus, it's going to float around there for a little bit before nucleases chew it up and you have to do the treatment again. You NEED some way to actually insert it into an already existing chromosome--this is where restriction endonucleases used to be the go-to method, and where CRISPR now outshines them by allowing you to insert it wherever you want instead of in predefined locations restricted by the specific restriction endonuclease you're using.
I honestly hope that CRISPR starts getting serious work done to try to advance the field of gene therapy, because a lot of those diseases are basically death sentences for people, and it looks like we have almost all of the pieces to start saving them instead.
I don't know if my wall-o-text adequately answered questions you may have had, but I hope it helped give you avenues to look further into or a better understanding of how the processes work and has helped you in some way. PubMed is always a great place to start reading for these topics also. Some stuff is behind paywalls, but some isn't.

It was very helpful and a great insight of medical work based on biology and chemistry. I myself study mechanical engineering with material science. We have the same delayed handout of materials to airplanes for example. It takes 20 years of continuous testing to get a new material accepted in air traffic. We are flying in 20 year old technology.
I'm glad I don't suffer from cancer but I know a lot of people who do. I've got a very rare combination of a lot of pain syndromes. Cluster headache (aka suicide headache), chronic migraine, chronic tension headache, new daily persistent headache (which can be categorized as a chronic tension headache), trigeminus neuralgy in an early stadium and for the 3 other pain situations I only know the German name.
It's a really, really bad situation. My mother and aunt both had cancer and they would never ever tradr with me they said. Cancer brings pain, makes you lose parts of your body and maybe cost your life but there is a chance for cure. I have to chance. I've got to wait for the day to come when I can't stand the pain anymore. Crawling on the floor to get my oxygen mask, screaming, losing consciousness or being on the edge, losing the ability to speak, pressing my fist and fingers into my face to make my situation better with the result of a purple, blue and black face the day after. My wife has to see me like this and it hurts me the most. She simply can't help. She tries her best. So does my mother. I lost 44 pounds in weeks when it all started. Now with meds I got fat. Over 200 pounds.
It's so frustrating. I was an A student at university, I was good in sports, going to friends and family all the time.
I learned to live with it but sometimes when you think you felt the worst pain of your life, the next wave comes a month later. I never ever have a minute of pain free time. I have times where I'm OK with the pain but the pain spikes kill me. 7 years.

You do have another problem that your pain sources are so complicated and complex that you'd need to identify the genetic source of each one, and then have a gene therapy for each one separately. And it would take many years for each one to be developed. My mother has trigeminus and migraine chronically too, but those aren't genetic but structural, so it wouldn't even help those, sadly. My mother is receiving somewhat experimental treatment with botox on the side of her face with the trigeminus, and it actually works. Each injection set lasts for about 2.5-3 months, and when it wears off, she starts to have small instances, gets a new treatment and it stops. Maybe this could help you too, but here in Europe it's still quite experimental...

Migraine actually is genetic. In 2015 they discovered that it is and in 2017 they identified all corresponding genes. In the next few month there will be a treatment that will block certain receptors of a neurotransmitter that induces migraine. A German Doctor (Hartmut Göbel) has earned the highest medal of honor you can achieve as a civilian because he found it out and with an amazing and umcomparable worldwide work of many countries they finally found the genes. I was fortunate enough to meet him in person a few times. He was honest and told me about how my life is going to be so I can at least prepare. Not all that other bullshittery of doctors, physicians, "healers" which are all sure they will cure you. This man was straight up honest. And I'm glad.

I'm lucky living close enough to Switzerland so when the time comes that every single option is depleted I can go there and find peace. It's a difficult topic to tell it to your wife. She is the only one knowing.
I wish I could see more of life, get old, sit in a chair in the back of my garden and let the sun shine down on me. Things always end up different to what you expect. So I do currently live my life in the best possible way and I'm happy with it.

And as for migraines, that's amazing, but yeah, a cure is a decade or more away from market. But hopefully you would be a good candidate for a clinical trial. Although the cluster migraines might cause problems with that, though... My father had cluster headaches when he was in his late twenties to thirties (I'm 31), and had my first regular migraine a few months ago, so I'm also quite nervous if I will develop cluster migraines like my father did... His eventually went away, but...
It's good that you have that option, but I really hope you won't have to use it... Both for your sake and your wife's. It's a heavy burden to bear, and I hope they'll get a cure going sooner rather than later. And again, if you can contact the specialist, maybe you can join a clinical trial and get treated early...?
