Amazingly hardly anyone does. And a warm therapist would make the client feel comfortable about it. Either making it funny so we can laugh together, or covering the sound so as not to make it heard, or compassionately ignoring it should tie client be asleep. In fact it’s a complement in a way, because it shows the client is relaxing and the body letting go!
