You know this was one of the scenes Kubrick altered substantially from the novel, deliberately, because in the novel Alex is only 15 or 16 and the “girls” he gets to come back to his place with him are ten year olds whom he gets drunk/rapes. But it would have been so beyond fucked up to shoot said scene and still have the slightest ounce of sympathy for the main character, hence it was changed to suggest a far more age-appropriate (though he is still older than them in the film, and a terrible influence at that 😂 it doesn’t come off quite as despicable) & I believe all the actors were of legal age irrespective of how old their characters were meant to be
It may be a comment on Alex himself! Given his total selfishness, misogyny and love of the old in out, it may be that Alex genuinely can't comprehend how women can enjoy sex, since I doubt he has many meaningful conversations with women about it and doesn't care about anyone except himself.

That grin of his is priceless. You see it and know that something funny, but slightly twisted is at work in his head. The threesome scene serves to express the dichotomy of beginning Alex vs. ending Alex, where he experiences life and love and fun while Beethovan is playing in the background, compared to the end when he experiences despair and suicidal thoughts while Beethovan plays.
