I am building a flat slab in my house and the width of the slab will be around 7 meters from column to another, my question is what diameter of rod and the space between the rods ???

First, you laid the longer bars in the bottom mesh which is wrong I think, usually in slabs we lay down the shorter bars first, then placed the longer bars over the short ones.

Always we put bars on shorter direction first.
But here you put first bars on longer direction.

As it seems, there are not enough cover blocks. Some bottom reinforcement seem to be hanging in the air.

Yes in flat slab but in solid slab we put the main bars in short direction .
Why is additional reinforcement needed for the postive moment?
The positive moment in the midle of the spane is greater than the positive moment at the ends of the spane
