The problem with Qxd6 was not the move before 4;05 I am going to goand chip at this, is fine but you have to see that you chip at the support of your pawn chain. As you say it was mostly well played a few careless moments, though not necessarily the moments you think were careless all of them were telegaphed a few moves before. And even if you do calculate, you calculate ONE MOVE and then stop.
